Project Overview The Jeff Davis County Hospital Authority was established in 1959 because the community saw the need for a hospital in Jeff Davis County. Clyde Duncan Memorial Hospital opened August 8, 1963 with 26 beds. Soon all beds were full and there was a need for more space therefore the first expansion was completed in 1969 with the addition of 16 beds making the hospital a 42 bed facility. Again, due to high occupancy and the need for additional bed capacity in 1972 there was a second expansion adding 14 beds, Business Office, Laboratory and Conference Room. Jeff Davis Hospital now had 56 beds. In 1993 the third expansion was completed which created a new Emergency Department, Laboratory, Pharmacy, ICU, Chapel, ICU Waiting room, X-ray Department and Medical Records Department - the largest expansion thus far. This expansion eliminated six beds reducing the facility to a 50 bed hospital. In 1994 plans were started to become a Joint Commission accredited hospital and completed in 1998. In 2004 Jeff Davis Hospital became a Critical Access Hospital with 25 beds. B. Project Description - ADULT/GERIATRIC PSYCHIATRIC UNIT Psychiatric patients requiring hospitalization often have medical problems as well. It’s a complicated situation that calls for extraordinary measures. Jeff Davis Hospital would be able to respond with a comforting environment designed to put these patients at ease. Our care environment is designed to resemble a lovely home or inn, rather than a hospital. Behind the scenes, a specially trained multi-disciplinary team works to evaluate mental and physical needs, coordinate care, and apply our array of resources. Jeff Davis Hospital Adult/Geri-Psych Unit Scope of Work - The scope of work will be new construction of an Adult/Geri-Psych Unit south of the hospital with approximately 8,000 sq. ft. wing as follows: • Construction of a new 10 bed Adult/ Geri-Psych unit with associated staff and patient support space, dining area, activity/group areas, business office space and facility support spaces • Design Build Design & Construction of the In-Patient Adult/Geri-Psych unit totaling approximately 8,500 sq. ft. • Fixtures, Furnishings and Equipment for the In-Patient Adult/Geri-Psych unit totaling approximately 8,500 sq. ft. Request for Proposal (Architect / Engineer) continued C. Scope of Services For projects advertised with an appropriately developed Program of Requirements (POR), upon award of the Agreement, commence with Design. For projects without such a POR, upon award of the Agreement, commence by developing the Program of Requirements. This project has had a preliminary program of requirements generated and will require additional programming development. The preliminary program along with conceptual floor plan layout will be made available to the short-listed firms. CAD floor plans of the building and copies of the original construction documents will also be provided to the short-listed firms. There will be a non-mandatory pre-interview walk-through for the short-listed firms. The selected Architect/Engineer (DESIGN BUILD FIRM), as a portion of its required Scope of Services and prior to submitting its proposals, will discuss and clarify with the Owner and/or the Contracting Authority, the cost breakdown of the Architect/Engineer Agreement detailed cost components to address the Owner’s project requirements. As required by the Agreement, and as properly authorized, provide the following categories of services: Program Verification, Schematic Design, Design Development, Construction Document Preparation, Bid and Award Support, Conformed Documents, Construction Administration, Post-Construction, and Additional Services of all types. Selected DESIGN BUILD FIRM is to provide 3D computer modeling for design exploration and for creation of presentation renderings. Refer to the Georgia Department of Community Health’s HFR Design and Construction for additional information about the type and extent of services required for the standard Agreement can be obtained at the DCH website at https://dch.georgia.gov/hfr-design-and-construction. During the construction period, provide not less than 6 hours (excluding travel time) on-site construction administration services each week, including (1) attendance at progress meetings, (2) a written field report of each site visit, (3) on-site representation comprised of the DESIGN BUILD FIRM and its consultant staff involved in the primary design of the project, all having relevant and appropriate types of construction administration experience. For purposes of completing the Relevant Project Experience Matrix of the Proposal, below is a list of relevant scope of work requirements for this RFP: 1. Design/Build of In-patient Adult/Geriatric Psychiatric Units (include drawings and cost Estimate) 2. Previous experience in managing Healthcare Facility construction projects adjacent to facilities that need to maintain operation 3. Previous experience in Construction of Healthcare Facility projects adjacent to facilities that need to maintain operation 4. Engineering for infrastructure/building systems upgrades in renovations/additions 5. Previous experience working with State of Georgia DCH 6. Funding / Estimated Budget Total Project Cost: $ 2,483,082 State Funding: $ 0 Construction Cost: $ 1,934,759 USDA-RD Funding: $ 2,483,082 Estimated A/E Fee: $ 87,900 Other Funding: $ 460,423 NOTE: The DESIGN BUILD FIRM fee percentage for this project includes all professional design services, and consultant services necessary for proper completion of the Basic Services for the successful completion of the project, including but not limited to: review and verification of the Program of Requirements provided by the Owner, validation of existing site conditions (but not subsurface or hidden conditions), preparation of cost estimates and design schedules for the project. Fees may be negotiated and allocated for Additional Services (e.g., creation of a Program of Requirements, extensive evaluation or validation of site conditions, extensive pre-design investigations, code-required special inspection and testing, Quality Assurance testing during the construction period, and testing due to unforeseen conditions). E. Services Required The DESIGN BUILD TEAM shall be: • a registered architect holding a license and certificate of authorization issued by the Georgia Architects Board • a landscape architect holding a license and certificate of authorization issued by the Georgia Landscape Architects Board • a licensed State of Georgia Contractor who has worked with the following: o Healthcare Facility Regulation (HFR), a division of the Department of Community Health (DCH), who is responsible for health care planning, licensing, certification. o Assist in the preparation of the Preliminary Architectural Feasibility Report as required for Rural Development financed facilities. • a professional engineer or professional surveyor holding a license and certificate of authorization issued by the Georgia Engineers and Surveyors F. Anticipated Schedule PHASE I Work Starts (contract signed): July, 2017 Phase I Cost Estimate: August, 2017 PHASE II Notice to proceed: August, 2017 Construction Start: September, 2017 Punch Out/Commissioning: August, 2018 Occupancy: September, 2018 G. Evaluation Criteria for Selection • Demonstrated ability to meet Owner’s programmed project vision, scope, budget, and schedule on previous projects. • Previous experience compatible with the proposed project (e.g., type, size). • Relevant past work of prospective firm’s proposed consultants. • Qualifications and experience of individuals directly involved with the project. Interested DESIGN BUILD FIRMs are required to submit the Commitment to participate by providing a Letter of Intent to the RFP, to indicate its intent to contract.
