Attachments: Camrose PCN Town Halls 2016.pdf Good afternoon, As you may know, the Camrose Primary Care Network (PCN) is preparing for its 2017-20 Business Plan submission. I am pleased to provide you with the following information and invitation to participate in one of our upcoming town hall and/ or community stakeholder consultation sessions. An essential part of the Camrose PCN' s vision is to: meet the priman; healthcare needs of Cam rose and Area through collaborative, comprehensive team based patient care. At present, the Camrose PCN has forty six (46) rostered physicians practicing in Camrose, Daysland, Forestburg, Bashaw, Hardisty and Tofield. The PCN also employs, seconds and/ or contracts 30 health professionals and adminish·ative staff. Detailed information regarding our PCN can be found at www.camrosepcn.com. Within our current Business Plan - the Camrose PCN identifies 5 priority initiatives: Prevention and Chronic Disease Management, Women's Health, Obstetrics and Newborn Care, Geriah·ics, Palliative and End of Life Care and Medical Capacity. Each initiative has a number of programs and services attached that are either lead by the PCN or in partnership with community stakeholders. Town Hall Sessions In October of this year the Camrose PCN will host 5 Town Hall meetings in Bashaw, Camrose, Daysland, Hardisty and Tofield. These sessions are meant to inform community members of the current PCN menu of programs and services as well as seek input regarding the patient's experience, satisfaction and outcomes as it pertains to primary care in their community. The Camrose PCN values all of our community partners and wishes to extend an invitation to Council to participate in one of our upcoming community town hall sessions. Dates and locations are as follows: October 12th 6:30pm-8:30pm Bashaw Majestic Theatre October 13th 6:30pm-8:30pm Daysland Community Centre October 17th 6:30pm-8:30pm Hardisty Community Centre October 18th 6:30pm-8:30pm Camrose Public Library October 19th 6:30pm-8:30pm Tofield Community Centre Stakeholder Sessions For your information stakeholder sessions will also take place starting in September with a half day scheduled for each initiative from 9am - noon at the Camrose Recreation Centre (Community Room Upstairs). The calendar is as follows: September 21st 9am-noon Palliative and End of Life Care October 20th 9am-noon Geriatrics October 24th 9am-noon Prevention and Chronic Disease Management October 25th 9am-noon Women's Health, Obstetrics and Newborn Care The first hour will be used to review the PCN' s current Business Plan and menu of programs and services. The second hour will be focused on a SWOT (Strengths, Weaknesses, Opportunities, and Threats) exercise. Lastly we will put together a summary of what we heard during the morning and highlight any suggested recommendations regarding partnership opportunities for existing programming as well as new opportunities. Patient Experience Survey 1 The Camrose PCN will also - once again - be conducting a patient experience survey that will be made available to individuals throughout our catchment area in hard copy and online to complete (Month of October/November). The survey will be similar to the one conducted in 2013 and I am confident it will produce the same successful return rate. Participation With the business plan renewal process in full swing I will not be scheduling time to present at each Council within our catchment area this Fall. I hope that you will consider participating in a town hall session near you. I would certainly appreciate having you in attendance. If I could also ask for your assistance in forwarding this information and invitation on to those individuals you feel are most appropriate to attend our town hall and/ or stakeholder sessions.
