SCOPE This plan applies to all South Central College employees/students. Contractors or outside personnel shall ensure their employees operate the equipment in compliance with safe work practices and standards, and are properly trained and certified. Contractors failing to adhere to applicable safe work practices and standards will be asked to terminate their work until their program is brought into compliance. DEFINITIONS Authorized Operator: An employee/student who has satisfactorily completed both classroom and operation training on material-handling equipment at South Central College. Load Center: The horizontal distance from the edge of the load (or the vertical face of the forks or other attachment) to the load’s center of gravity. Rated Capacity: The maximum weight that the powered industrial truck is designed to lift, as determined by the manufacturer. RESPONSIBILITIES Security & Safety Director Establish, maintain, and update the program. Coordinate forklift training and re/certification for employees. Individual instructors will be responsible for assuring any student is properly trained as required. Maintain training documentation. Maintenance Supervisors Service and/or repair equipment in a timely manner. Contact outside vendor for repair as needed. Ensure equipment is not put back in service until necessary repairs have been made and is in safe operating condition. Maintain maintenance records for each forklift. Records shall be kept for the life of the equipment. Managers, Supervisors, and Faculty of Department with Assigned Forklift Implement and administer this plan as it applies to the department. Ensure employees/students in the department follow all rules and procedures pertaining to this plan. Do not allow anyone to operate the equipment without proper training and a current certification from South Central College. Do not allow people outside of South Central College to operate South Central College owned powered industrial equipment. Maintain forklift pre-operation inspection checklists. Checklists should be kept for at least one year or until noted issues have been corrected, whichever is longer. Contact the maintenance supervisor of the appropriate campus to arrange for servicing and repair. Do not allow anyone to operate malfunctioning equipment. If there is a condition affecting the equipment’s safety or operation, ensure an out-of service tag is placed on the equipment’s steering wheel. Ensure equipment is not placed back in service until the safety concern has been addressed. Report all forklift incidents/accidents to the appropriate campus maintenance supervisor and the Security & Safety Director immediately. Employees/Students Do not operate the forklift without proper training and a current certification from South Central College. Perform pre-operation inspection of forklift prior to operation and turn in Pre-Operation Inspection checklist to the campus appropriate maintenance supervisor or faculty member. Place “out-of-service” tag on forklift’s steering wheel if there is any condition affecting the safety or operation of the equipment and notify your supervisor/faculty member of the disorder. Do not operate equipment or remove “out-of-service” tag until the safety concern has been corrected. Report any deficiencies or issues with the equipment immediately to your supervisor/faculty member for repair/servicing. Do not tamper with the equipment yourself. Do not use malfunctioning equipment. Maintain good housekeeping and do not abuse the equipment. Comply with departmental instructions and procedures for checking equipment in/out. Follow all rules and procedures pertaining to this plan. Report all forklift incidents/accidents to your supervisor/faculty member immediately. TRAINING Employees/students must satisfactorily complete training and be certified by South Central College before being authorized to operate the forklift. Employees/students who received forklift operator training through a prior employer/source must still complete initial training at South Central College. Employees/students who were trained or certified on a different type of forklift other than the type they are assigned to use are required to complete training on the “new” equipment. The training only needs to include the practical and operator’s performance evaluation, unless the trainer determines that the classroom portion also needs to be included. Operator training, evaluation and certification shall be conducted by designated personnel who have the knowledge, training and experience to train powered industrial truck operators and evaluate their proficiency. Training will be coordinated through the Security & Safety Director. A department can request training for an employee/student by contacting that office. Initial and refresher training will be provided as outlined in Appendix A. Training will consist of a classroom portion as well as hands-on equipment operation and practices. Upon successful completion of training, employees/students will be certified by the trainer and be issued a certificate of completion.
