GENETICS AND GENODERMATOSES A CASE OF ACQUIRED PENETRATING DERMATOSIS Ling Li (1) School Of Medicine Of University Of Electronic Science And Technology, Department Of Dermatology,institute Of Dermatology And Venereology, Sichuan Academy Of Medical Sciences & Sichuan Provincial People's Hospital, Chengdu, China (1) Background: Acquired penetrating dermatosis includes: creeping perforating elastic fiber disease, reactive penetrating collagen disease, perforating folliculitis and Kyrle's disease. This disease is often accompanied by chronic renal failure and one systemic disease at least. Observation: A 69-year-old woman with erythema, plaque, erosion and pain on her left face was admitted to the hospital for 3 years. Three years ago, a bright red patch of nail cap size appeared on her left side face, accompanied by slight pain, itching, and suspicion of Discoid Lupus Erythematosus.Two biopsies of left facial skin lesion suggested that HPV infection of squamous epithelium pseudodermoid hyperplasia and acid-fast staining was negative.The patient were admitted to the ward with DLE and contact dermatitis, Patient suffer from nephrotic syndrome for 30 + years, hypertension for 20 years, hypothyroidism, Hashimoto's thyroiditis for 10 + years, Skin examination: Dark brown-red plaques with obvious infiltrationon on left cheek, hard nodules, epidermis loss, erosion and reddish granulation tissue were observed,beans to coins dark brown patches on the back of the nose, with pigmentation on the edges.The pathological biopsy of facial skin showed that the skin tissues were hyperkeratosis and keratosis, no liquefaction of basal cells and pigment incontinence, hyperplasia of dermal fibers, and infiltration of chronic inflammatory cells around blood vessels. Diagnosis: acquired penetrating dermatosis. 0.1% tacrolimus bid, red light irradiation on facial skin lesions, the patient was discharged from hospital with change for the better.
