EMERGENCY CONTACTS • NHS 24 – Offers you a range of telephone services. You can get advice on health issues, together with details of local GP, dental and pharmacy services in your area. NHS 24 also offers personal medical advice and guidance. Should you need urgent help, NHS 24 staff may direct you to one of the accident and emergency or out-of-hours GP Services. Tel 0845 4 24 24 24. • ACCIDENT AND EMERGENCY – Your nearest accident and emergency department is located within Glasgow Royal Infirmary, 84 Castle Street, Glasgow, G4 0SF, tel 0141 211 4000. • OUT-OF-HOURS GP SERVICES – The Greater Glasgow NHS out-of- hours service for North Glasgow is based at Stobhill Hospital, 133 Balornock Road, G21 3UW. If you dial your own GP surgery telephone number you will automatically be diverted to this service. • SOCIAL WORK STAND-BY SERVICE – Social Work operate an emergency out-of-hours service from 4:45 p.m. – 8:45 a.m. Monday to Friday and 3:55 p.m. Friday to 8:45 a.m. Monday (Weekend). INTRODUCTION – Alex MacKenzie, Director North Glasgow CHCP Community Health and Care Partnerships, or CHCPs for short, are the new organisations which are being introduced across Glasgow to manage a wide range of community-based health and social care services. CHCPs will further improve the way our local community health and social care are organised and managed. They will develop closer partnerships between community health, social care and specialist hospital based services. They will also take a lead in improving the health and well-being of local people by working closely with local housing, leisure and education services, community and voluntary organisations and organisations responsible for training and employment. Your new CHCP will be responsible for a wide range of health and social care services provided in local health centres, clinics and Social Work offices across North Glasgow. These include services provided by health visitors, social workers, district nurses, school nurses, community physiotherapist, dieticians and podiatrists (foot care specialists). The CHCP will also be responsible for local learning disability, mental health and addiction services in the North of the city and will work closely with local GPs, pharmacists, dentists, opticians and specialist staff from local hospitals. The new North Glasgow CHCP came into force on the 1st of April 2006. In the first year the emphasis will be on: • Ensuring that patients, carers, members of the public, community and voluntary bodies and staff have a voice in the design and delivery of services. • Producing an agreed plan for the future development of local health and social care services. • Helping staff from health and Social Work services work closer together. HOW CAN YOU GET THE RIGHT HEALTH AND SOCIAL CARE? Everyone in Glasgow will have contact with health or social care services at some point in their lives. This could mean seeing your GP or dentist, or could involve an older family member in need of extra support to stay at home or a child in need of support or protection. What services can you use? • CHCP Services – your CHCP offers a wide range of health and social care services, mostly within office hours. A list of these services is given in the A-Z of services on pages 8-10. Health services are available through GPs, dentists, pharmacists and optometrists across North Glasgow. There are also six community health centres with specialist services and clinics. You can go to any of these health centres to access the services there. There is no charge for most health services. You may be liable to some charges for some prescriptions, dental services, hearing aids and glasses. If in doubt about charges, please ask a member of staff. Social care services are usually accessed through the local Social Work office. If you have an emergency within office hours, we will give you an urgent appointment. Outwith office hours, phone our Standby Service on 0800 811 505. Apart from initial information and advice, it is usually necessary to be assessed for a social care service. There is no charge for assessment but for some services, for example home care, there may be a charge but this normally depends on your income. • Partners’ Services – North Glasgow CHCP works in partnership with local housing, leisure and education services, community and voluntary organisations responsible for training and employment. Tel: 0141 332 2444 or www.infobaseglasgow.org • Getting and Staying Healthy – Lifestyle, particularly a healthy diet and exercise, is one of the main contributors to good health. Attitudes to alcohol and drugs (including smoking) will also greatly affect health. Stress, or more importantly the lack of it, will influence mental well being. For details of local health improvement projects in North Glasgow contact the Health Improvement Team on 0141 201 9707. • Helping us to help you and others: All of our services are greatly in demand. Please use them sensibly, in particular emergency and out-of-hours facilities. If you cannot attend an appointment, please cancel it so the appointment can be offered to someone else. Failure to attend appointments costs thousands of pounds each year and means other patients have to wait longer. If you think you no longer need a hospital appointment, please speak to your family doctor first.
