Medication Adherence - Diabetes Medications, Renin Angiotensin System (RAS) Antagonists, and Statins MEASURE DESCRIPTION Medication Adherence for Diabetes Medications Percent of Members 18 years and older with a prescription for diabetes medication who fill their prescription 80 percent or more of the time. Diabetes medication include biguanides, sulfonylureas, thiazolidinediones, and DiPeptidyl Peptidase (DPP)-IV Inhibitors, incretin mimetics, meglitinides, and sodium glucose cotransporter 2 (SGLT) inhibitors. Members who take insulin are not included. Medication Adherence for Hypertension (RAS antagonists) Percent of Members with a prescription for a blood pressure medication who fill their prescription 80 percent or more of the time they are supposed to be taking the medication. (Blood pressure medication means an angiotensin converting enzyme inhibitor (ACEI), angiotensin receptor blocker (ARB), or direct renin inhibitor medications. Medication Adherence for Cholesterol (Statins) Percent of Members with a prescription for a cholesterol medication (a statin drug) who fill their prescription 80 percent or more of the time they are supposed to be taking the medication. To view Molina Healthcare’s formulary, please visit our website at www.MolinaHealthcare.com TIPS TO IMPROVE MEDICATION ADHERENCE Provide short and clear instructions for all prescriptions. Consider providing prescription instructions in both English and Spanish for Spanish-speaking patients. Encourage the patient to take medications at the same time of day. Encourage the use of a pillbox or a calendar to help patients to take them each day. Schedule a follow-up visit when prescribing a new medication to your patient within 30 days to assess how the medication is working. Schedule this visit while your patient is still in the office. Adjust the timing, frequency, amount and dosage when possible to simplify the regimen and consider offering a 90 day supply. Inform the patient of the benefits of taking the medication as directed and the importance of continued use. Make sure they understand the consequences of not taking the medication.
