This handbook does not exempt the applicability of OSHA requirements for contractors and sub-contractors but reinforces them. Contract work for UT Austin does not exempt the applicability of OSHA requirements. 2 EMERGENCY CONTACTS Fire, EMS or Police 911 UT Police 512-471-4441 UT Facilities Service Center 512-471-2020 UT Parking and Transportation Services 512-471-7275 UT EHS 512-471-3511 3 INCIDENT REPORTING All contractors must report any near miss, damage to UT property, and injury beyond first aid to the contractor supervisor to the UT Austin hiring individual. After notifying the UT hiring individual, the contractor supervisor must contact UT Austin EHS. All incidents must be reported to EHS within 30 minutes of occurrence. In the event of any OSHA defined incident, the contractor is still obligated to comply with OSHA reporting requirements. OSHA inspectors are welcome to UT campus at any time and could be present during special events. 4 SPILL PREVENTION, RESPONSE, AND EROSION CONTROL Immediately notify UT Facilities Service Center of any spills of chemical/petroleum liquids or blood. Have your name, phone number, location, and event information available at the time of the call. Do not discharge anything to the creek or storm sewers Proper erosion and sedimentation controls must be in place to prevent sediment or silt run-off. Sediment must never be rinsed off at the site. Instead, sediment must be collected in a manner that does not allow it to reach a storm drain or Waller Creek. Avoid tracking sediment into the roadway or off the site. Bags containing sand are not authorized for use on the UT campus. 5 HOUSEKEEPING Pedestrian pathway closures for events must be pre-approved through the event coordinator and by campus. Pedestrian walkways and ADA pathways (i.e., wheelchair access) must be maintained and protected. Approved temporary pedestrian pathway closures must include physical barricades, on-duty pedestrian traffic guard (when applicable), and signage to reroute pedestrians. The work areas and all premises occupied by the Contractor and associated subcontractors must be kept clean, healthy, and sanitary at all times. Work areas, passageways, and structures must be kept clear of debris and trip hazards. Construction materials must be stored in an orderly manner. Site storage areas and walkways shall be kept free of dangerous depressions, obstructions, and debris. Construction equipment shall be stored in an orderly manner in a pre-approved location. The Contractor will be held responsible for the immediate cleanup of any adjacent areas should the work infringe into unauthorized areas of construction. 6 ELECTRICAL SAFETY AND CONTROL OF HAZARDOUS ENERGY All work requiring the isolation of energy requires a Control of Hazardous Energy procedure. The Contractor shall provide all lockout tagout equipment to complete the work. The Contractor must communicate with all others in the area that could be affected. Revised 5/14/2019 1 Extension cords used with portable electric tools and appliances shall be UL rated, 12 AWG, heavy duty (S, SO, STO, SJ SJO, SJTO) and of the three-wire grounding type. The cords shall conform to the type and configuration required by OSHA standards and be in good working order. The cords shall be used with GFCI Plus adapters or outlets. 7 FORKLIFTS, BOOM LIFTS, CRANES, SKYTRAKS Overhead work (boom lifts, cranes, skytraks, etc.) with material handling equipment must comply with the following and require pedestrian protection. A physical barricade is required to prevent pedestrians from encountering overhead work. A pedestrian traffic control guard is required to prevent pedestrians from crossing the physical barricades. All cranes must follow the UT Austin campus mobile crane program and procedures. Boom lifts and Skytraks must be included in the daily Job Hazard Analysis (JHA) and requires proof of training. Manufacturer’s requirements including lifting capacities based on configuration for each piece of equipment must be followed. Contractors shall not use shop made or altered tools and equipment unless approved by the manufacturer or stamped by a Professional Engineer. The contractor’s inclement weather plan must address wind speed limit and lightning range while operating equipment during setup, event, and strike. Keys cannot be left in the ignition of vehicles when not in use. 8 PEDESTRIAN PROTECTION, SECURING EQUIPMENT, SITE CONTROL AND BARRICADES Physical channelizing devices/horizontal barricades and a dedicated pedestrian traffic guard present during all overhead activities are required. The horizontal barricades must be placed away from the work area to prevent falling objects/workers from striking pedestrians and occupied vehicles. The pedestrian traffic guard must stand at the barricade and prevent pedestrians from entering the exclusion zone. Pedestrian access must be restricted at all times from the barricaded work area. When work moves beyond the barricaded work area, flaggers and mobile barricades must be used. The Contractor will be held responsible for any pedestrians entering the work area. Physical barricades must be utilized effectively and not be easily moved or bypassed. Signage is required when pedestrian access is restricted. Signage must provide adequate information redirecting pedestrians around the work area. Work areas must be further secured after hours to prevent access and/or vandalism. The Contractor shall ensure that all project workers secure their tools. Any thefts should be reported to UTPD and their UT hiring individual. Keys cannot be left in the ignition of vehicles when not in use. 9 HOT WORK Hot Work is defined as any type of open flame welding, cutting, or soldering that takes place on a project. Any hot work must be pre-approved by UT Austin Fire Marshal. 10 IDENTIFICATION AND BADGING OF WORKERS All job site personnel shall have identification or Contractor issued badges available. This requirement is in addition to the safety vest requirement for all workers while on campus. 11 SCAFFOLDING All scaffolding must be installed and inspected under the supervision of a scaffolding competent person. Each individual scaffold must be tagged by the competent person prior to use. Revised 5/14/2019 2 12 HEALTH AND SAFETY PLANS Safety related documentation for construction work shall be retained in a job site binder. The binder shall include as a minimum the following documents: • Site Specific Health and Safety Plan • Job Hazard Analysis/Job Safety Analysis (JHA or JSA) • Project Roster • Contractor and Subcontractor Emergency Contact Information • Shelter in Place Plan (may be incorporated with the Evacuation Plan in an Emergency Plan brief) • (Material) Safety Data Sheets • Evacuation Plan (may be incorporated with the Evacuation Plan in an Emergency Plan brief) • Map to nearest 24 hour emergency medical facility • Inclement Weather Plan All workers are required to take water breaks. All workers must be able to identify the signs of heat stress and respond accordingly (information must be provided in Site Specific Health and Safety Plan). 13 STOP WORK AUTHORITY Throughout all phases of work, UT personnel will monitor field activities on a regular basis to ensure that work is being conducted safely. Observations by OSHA may take place at any time to ensure compliance with applicable codes, standards, and regulations. All UT personnel
