Minimum Work Hours You must be working a minimum of 32 hours per week to be eligible for coverage. Required Coverage Payment Your employer pays 100% of the premium for this coverage. GUARANTEE ISSUE AMOUNT(S) All benefit amounts outlined below are Guarantee Issue. No evidence of insurability is required.
