Chamorro Standard Time (ChST) DATE: Tuesday, March 28, 2017 To: TIME: 9:00 a.m. Chamorro Standard Time (ChST) DATE: Friday, March 31, 2017 Page 19, Section 3.1.16 — BOND REQUIREMENTS, PERFORMANCE, AND PAYMENT GUARANTEES Add the following paragraph: PERFORMANCE BOND Pursuant to 2 GAR Div 4 §3109(c)(4), the successful bidder is required to deliver a performance bond executed by a surety company authorized to do business in Guam for one hundred percent (100%) of the contract price. The purpose of the performance bond is to indemnify GDOE from loss resulting from the failure of the bidder to perform the services in accordance with requirements of this IFB and any contract resulting from this IFB. The performance bond shall be in the form contained in this IFB (GDOE Procurement Form 003). Failure to deliver the required performance bond shall result in the rejection of the bid and collection of the security for the rejected bid. ** Attachment 1 in this Addendum contains GDOE Procurement Form 003 — Performance Bond. This form is not required until after receiving Intent to Award. ATTACHMENT B — School’s Wireless Network Installation Specifications (SWNIS) Replace the following schools’ SWNIS and Floor Plan with the attached revised SWNIS and Floor Plans: 1. Replace Merizo Martyrs Elementary Schools SWNIS Revised #1 with the attached SWNIS Revised #2 (Attachment 2). Changes were made to the room numbers to identify the reassigned classroom numbers for the WAP installation and room number for the location of the IDF. Summary ofAdditional Site Visit on Friday, March 3, 2017 Addendum No. 3 IFB 010-2017 E-RATE INTERNAL CONNECTIONS SERVICE — WIRELESS LOCAL AREA NETWORK (WLAN) INSTALLATION Page 2 of 4 Please see below answers to questions submitted by Prospective Bidders: Banning Contracting Services, Inc. 1. Will there be an area set aside for staging material and large equipment? If yes, where? GDOE Response: No. The Bidder is responsible for the facility to stage materials, supplies and large equipment required to complete the WLAN installation at 39 public schools. The Bidder shall clean the work area, remove all unused materials, supplies, and equipment used for the installation at the end of each work dayfrom the school site. 2. Do you have an anticipated award date and anticipated start date for this project? GDOE Response: The GDOE is seeking E-rate funding in E-rate Cycle Year 2017. The E-rate application window will close on May 11, 2017. The intent to award and/or contract award must be issued prior to the submission of GDOE’S E-rate Year 2017 application. The start date for this project is dependent upon the issuance date of an approved Funding Commitment Decision Letter from USAC E-rate Program and thereafter the issuance date of the Notice to Proceedfrom GDOE. 3. Do you have an estimated or required period of performance? GDOE Response: The estimated period of performance is six (6) months from the issuance of the Notice to Proceed (NTP). 4. Do you have a preference on how the cabling is labelled? GDOE Response: The label shall identify the WAP and classroom number. 5. Does MO Conference Room have data drops, or do we need to run CAT 6 cable? GDOE Response: Yes, the room has a data drop similar to other classrooms in J. F. Kennedy High School campus. 6. Does D102 have data drops, or do we need to run CAT 6 cable? GDOE Response: Yes, the room has a data drop similar to other classrooms in J. F. Kennedy High School campus. 7. Do you have a preference for conduit type, brand, etc.? GDOE Response: The preferred conduit type from the IMO data drop to the WAP is raceway conduit. The raceway conduit shall be fasten and secure to the wall by screws. The conduit type for the room without IMO data drop is preferred to be PVC conduit that will run from the room to the nearest IDF or to the MDF if closer. The design and installation of conduits and pathway shall conform to TIA/EIA 569 pathway specification. G4S Security Systems (Guam) Inc. 1. G4S is humbly requesting the deadline for the submission of bid extend from March 28, 2017 to April 11, 2017. GDOE Response: GDOE is extending the deadline for submission to Friday, March 31, 2017, at 9:00 AM as stated above. Extending the submission any further will have an impact on the bid’s post activities timeline such as: bid’s evaluation; Bidder’s clarification; contract legal review; contract review by the Attorney General Office and Governor Office which affect the timeline to file GDOE’S E-rate 2017 application to fund the project. GDOE Response: Since this is currently an open procurement, GDOE cannot provide the budgetfor this project. 3. Is this Federal fund project? What is the percentage? When does it expire? GDOE Response: GDOE will be seeking E-rate funding for the eligible pre-discount amountfor the project on the Bid’s awarded amount. The E-rate Program funding percentage for the project is 85% of the eligible pre discount amount and GDOE share of the amount is 15%. GDOE willfund 100% of the total amount for the products or services that are not eligible for E-rate funding. The non-recurring service for E rate Funding Year 2017 ends on September 30, 2018. 4. Was this bid conducted last year? If yes, who was it going to be awarded to? What was the reason for rebidding? May we obtain copy of the protest document or bid documents from the previous bid? GDOE Response: Similar bid was conducted last year for WLAN installation to expand wireless coverage at 13 secondary schools. The bid was not awarded due to a protest that was filed, with an Appeal to the OPA thereafter. and time had lapse surpassing the deadline window to file an E-rate Funding Year 2016 application to provide discount funding for the project. A copy of the protest document or related bid documents may be obtained through a Freedom of Information Act (FOIA) Letter requested to GDOE’S Office ofSupply Management. 5. On page 15, section 3.1.3 Multi-Step Sealed Bid, The Bid Bond Requirement as stipulated in Section 3.1.18 — Bond Requirements, Performance, and Payment Guarantees must be submitted with the Priced Bid. There is no Section 3.1.18 in the bid package; this is identified as Section 3.1.16. Performance Bond and Payment Guarantees are also not included in the bid package. Is this an error? Normally these documents are provided after intent of award is provided to the contractor? GDOE Response: Please refer to addition to Section 3.1.16 stated on the first page of this Addendum. 6. There is a 100% performance bond required for this bid upon award? What triggers the performance bond? GDOE Response: Please refer to addition to Section 3.1.16 stated on the first page of this Addendum. 7. What government occupation codes and hourly wage rates does this bid apply to? GDOE Response: Please refer to original IFB’S “Declaration re Compliance with U.S. DOL Wage Determination” Form and the “Register of Wage Determinations under the Service Contract Act”. It is the responsibility of the Bidder to determine the occupations required for this Project and to comply with the U.S. DOL Wage Determination. 8. What version of Airwave is currently installed?
