December 22, 2017 RE: Oakland International Airport – 2018 Ground Transportation Limousine Permit Application Package To Whom It May Concern: Thank you for your interest in providing ground transportation services at Oakland International Airport (OAK). The 2018 Ground Transportation Permit application is attached for your convenience. When submitting your application package, please ensure that copies of all requested documents are attached to the application. Please allow ten (10) business days for your application to be processed. Incomplete applications, including applications lacking any required attachments, will not be processed and will be returned. Deadline: The deadline for submitting Limousine Permit Application Packages to the Ground Transportation office is May 31, 2018. New and Renewal Applicants: All applicants must submit an application package with the required documents and the non- refundable application fee. Companies that are currently operating at OAK should complete the Renewal Application in the attached package and submit the required documents, along with any updated information required in the application package. Renewal applicants are not required to complete a New Application package. Applications are available online at www.oaklandairport.com/permits/. Applications must be approved by the Airport or your company will not be authorized to operate at OAK. Companies with outstanding fees cannot obtain permits. Application Submittal: If you need assistance with the completion of your application package, please schedule an appointment by calling (510) 563-2897. Appointments are required to meet with Ground Transportation Staff. Office hours are 9:00 a.m. to 4:30 p.m. Monday through Thursday. Permit applications can be submitted in person or by mail: 1. Drop-off: No appointment is necessary to drop off completed application packages, which can be dropped during business hours at the Ground Transportation office counter; or during off hours by depositing your completed application into the drop box located on the left side of the ID Badging office, which is next to the Ground Transportation office. Look for the white drop box built into the left-side wall. 2. Mail: Application packages can be mailed to: Ground Transportation Office, Oakland International Airport, One Airport Drive, Box 45, Oakland, CA 94621. Submitted applications must include: Completed application package Payment for application fees – Credit card, business check, money order, cashier check Transponder fee for each vehicle – $50.00 per vehicle (see GT Fee Schedule) Driver Roster - Full name, address, telephone number of each driver (see attached) Copy of current and valid TCP or PSC certificate Vehicle Equipment statement – list all vehicles to be included in the permit to operate. Include transponder numbers for any San Jose transponder already installed on each vehicle (see attached) Copy of current vehicle registration Documentation of CHP inspections Schedule of fares to be charged to passengers (if applicable) Evidence of Insurance – each qualified operator shall provide Accord insurance certificate (see attached) Designated Insured Endorsement (see attached) Commercial General Liability and Automobile Liability Worker’s Comp (if you have employees) Sub-Carrier Company (if any) Required Information: Sub-Carrier Application (for Sub-carriers providing service under someone else’s, (applicant’s) business name) Notarized copy of contract between applicant and sub-carrier Sub-carrier submits notarized release from agreement from any other carrier Copy of current and valid TCP or PSC certificate Copy of current vehicle registration Evidence of Insurance – each qualified operator shall provide Accord insurance certificate (see attached) Designated Insured Endorsement (see attached) Commercial General Liability and Automobile Liability Worker’s Comp (if you have employees) Page 2 of 3 Incomplete applications will not be processed and will be returned. Once the application review process is complete, you will be contacted to have your vehicle(s) inspected by Ground Transportation staff. All payments must be made by credit card or with a bank-cashier’s check, business check, or money order, payable to the Port of Oakland. Personal checks will not be accepted. Automated Vehicle Identification (AVI) Transponder Requirements: AVI transponders shall be installed on all vehicles that operate at Oakland International Airport. Airport Rules and Regulations (Port Ordinance 4445) Section 8.1(e) requires that each Qualified Operator, Partially Exempt Qualified Operator, and Exempt Qualified Operator shall pay a non- refundable $50 Operator Transponder Fee per vehicle. This fee shall be returned only if the applied-for permit is not issued. Insurance Requirements: Insurance requirements are specified in the application package and proof of same shall be submitted in the application and mailed to the Risk Transfer Office, Oakland International Airport, One Airport Drive, Box 45, Oakland, CA 94621. The appropriate level of insurance must be provided with the proper endorsements as specified. OAK Ground Transportation Permits will not be issued if these amounts are incorrect. Plan B - Partially Exempt Qualified Operator. This plan is for operators who make 60 trips per year or less. No Security Deposit is required and Operator cannot operate a Courtesy Vehicle, Bus and/or any Vehicle over 30 feet long. $ 222 Application Fee Plan C - Exempt Qualified Operator. This plan is for operators who make 120 trips per year or less. No Security Deposit is required. Operator cannot operate a Courtesy Vehicle, Bus and/or any Vehicles over 30 feet long and makes no more than 120 trips per year. $ 444 - Application Fee The applicant acknowledges the application fee submitted does not include any required Security Deposits and/or Transponder Fees. Regardless of the contractual relationship between the individual driver and the company, the company will be responsible for all fees and charges incurred by drivers and the company will be billed accordingly.
