Rochester Regional Health Leave of Absence Returning to Work from a Continuous Leave of Absence How to return to work from a disability leave or a continuous leave under the Family and Medical Leave Act (“FMLA”) for a reason related to your own illness or injury: If you are returning from a disability leave or a continuous FMLA leave due to your own illness or injury, you must be cleared by Employee and Occupational Health (“EHS”) prior to returning to work. To submit appropriate medical documentation from your medical provider in order to be cleared to return to work, the documentation should be emailed to: [email protected]. • Please note that EHS will review the medical documentation provided and prepare a clearance note, which will then be sent to RRH Leave Administration and your direct supervisor. Upon receipt of the clearance note provided by EHS, Leave Administration will return you from leave in Workday, which will trigger a reinstatement of your access by IT. • Please note that if you have not been cleared by EHS, you will be unable to return to work in any capacity and your access may remain suspended until you are cleared. If the return to work clearance contains restrictions, Leave Administration must be notified by your direct supervisor whether or not restrictions can be accommodated before access can be re-enabled. How to return to work from a continuous leave under the New York Paid Family Leave (“PFL”) or FMLA laws for a reason unrelated to your own illness or injury: If you are returning from a continuous leave of absence under PFL or FMLA and the reason for the leave is related to something other than your own injury or illness, you must provide advance notice to your direct supervisor, Lincoln and RRH Leave Administration of the intended return to work date. Advance notice of one (1) week is requested in order to ensure timely return access. A medical release to return to work is not required.* • Please note that if you do not contact Leave Administration regarding your return from leave, your return to work may be delayed and access restricted until proper notification is received. * If you are out on a PFL leave that begins directly after a disability leave due to the birth of a child, you must be cleared by Employee and Occupational Health prior to returning to work.
