Based on the prima-facie causes of consequential train accidents which took place recently, following “ALERTS” are advised for taking necessary precautions to pre-empt similar occurrences. Rolling back of Light Engine on a falling gradient:- Rolling back of one of the two engines of couple light engines during shunting due to the shunting staff and the shunter failed to insert locking pin of CBC at a station situated on a falling gradient 1-136 on Guntakal Division of South Central Railway. Derailments due to track defect:- Derailment of an EMU local train on Mumbai Division of Central Railway due to excessive twist and tight gauge. Derailment of a goods train on Nagpur Division of South East Central Railway due to cross-level variation at deep screening site. Derailment of an express train on Dhanbad Division of East Central Railway due to poor maintenance of track. Derailment owing to non-observance of Signal aspect by Loco pilot:- Derailment of a goods train on Vijayawada Division of South Central Railway due to overshooting of Starter signal by Loco pilot. 1 Accident at Manned Level crossing Gate as the Gateman failed to close the gate even after giving assurance to the Station Master supported by private number:- Dashing of a truck against an express train at manned level crossing gate on Sambalpur Division of East Coast Railway as Gateman failed to close the gate even after giving assurance of closure of gate to the Station Master supported by private number. Derailment due to infringement caused by falling a girder on the track:- A goods train on Mysore Division of South Western Railway derailed due to infringement on the track. Clamps on temporary Fish plated joints over newly launched girder gave way due to improper fixing and use of old wooden sleepers. Fire in coaches due to electrical defects:- During the nine month period from March, 2009 to December, 2009, there have been 13 cases of fire in Coaches. The major cause has been defect in electrical wiring in 7 cases as per inquiry reports received from Zonal Railways. In addition, system of maintenance of Generator Car of Rajdhani/Shatabdi/Duranto trains and pantry cars (LPG fittings and electrical appliances) need to be set up on each Zonal Railways.
