Number of Maryland residents being notified: One Maryland resident was affected. Please see attached sample of the notice being sent to affected patient. The information compromised: Demographic information, including first and last name, medical record number, date of birth, phone number and email; Clinical information, including potential participation in a research study for gastrointestinal diagnosis. Steps taken: NM restricted access to the tablet computer as well as access to the shared drive via any tablet computer. A notice to the affected patient will be sent on June 13, 2019.
