LEASE AGREEMENT This Agreement made and entered into as of the ___ day of _________, _____, by and between the State of Tennessee, Department of Intellectual & Developmental Disabilities Services as Lessor, hereinafter called the “State”, and_______, as Lessee, hereinafter called the “Tenant”. The State and the Tenant agree as follows: 1. PROPERTY The State hereby leases to the Tenant for the term of this agreement: a. The property located at: 543 West Shelby Drive Street Number and Street Name Unit No. Memphis Tennessee 38109 City State Zip b. The Following Furniture and appliances: Washer, Dryer, Stove, Refrigerator 2. TERM The term of this lease shall commence on March 1, 2019, and shall end on December 31, 2020. 3. RENT Rent Amount is based on Tenant’s availability of funds, and ability to pay. The Tenant agrees to pay rent in the amount of $325.00 per month. Each payment is due on the 10th day of each month and to be sent to: State of Tennessee Department of Intellectual & Developmental Disabilities Services UBS Tower 315 Deaderick Street, 8th Floor Nashville, TN 37243 4. UTILITIES/SERVICES Utilities and services such as electricity, gas, water, and garbage collection shall be paid by the Tenant. 5. THE PARTIES ALSO AGREE: a. Tenant shall keep the premises in good condition and shall not make any alterations thereon without the written consent of the State. 1 b. Tenant shall be responsible for routine maintenance to keep the grounds in a clean, orderly, and neat manner (i.e., routine lawn care such as mowing and trimming shrubbery). c. Except for a. above, the State shall provide maintenance to the property as needed. This includes work necessary to keep the premises in a fit and habitable condition. d. Tenant shall, upon termination of this Agreement, vacate and return furniture and appliances in the same condition that it was received, less reasonable wear and tear, and other damages beyond the Tenant’s control. e. If the Tenant intends to renew the lease for an additional term, the Tenant shall notify the State of its intention at least 60 days prior to the expiration of this Lease. f. If during the term of this Lease, the leased premises are so damaged by fire, tornado, or other catastrophe or casualty as to render the same uninhabitable, this Lease shall terminate. g. Either party, the State and the Tenant, may terminate this Lease by giving the other party sixty (60) days written notice of their intention to do so, stating the day and date the termination will be effective. h. All notices herein provided to be given, or which may be given, by either party to the other, shall be deemed to have been fully given when made in writing and deposited in the United States mail, certified, and postage prepaid, and addressed as follows: State of Tennessee Tenant: Department of Intellectual & Developmental Disabilities Services 543 West Shelby Drive UBS Tower Memphis, TN 38109 315 Deaderick Street, 8th Floor Nashville, TN 37243 Any alternations to this Agreement shall be in writing and signed by all parties.
