Sweitz Dump Trucking VGB t/� 10/15/2007 I placed a phone call to Derek Wilson, Assistant District Fish Biologist, with ODFW at (541) 867-4741 regarding his comments to DOGAMI's note to reviewing agencies (21- 0065 12-06-06note ) . He suggested requiring an engineered stamp of approval that the sedimentation ponds were sized adequately to contain storm water runoff from 24 hour storm events. I explained that the 1200A permit did not have this requirement and that as long as the permittee met the required storm water benchmarks they would be in compliance with their permit. I stated that I would conduct a site inspection with in 1-2 months to observe the storm water system directly. VGB 2/22/2007 On August 11, 2006 DOGAMI received an application for an Operating Permit from James B. Sweitz Dump Trucking. During the initial processing of the application it was determined that the business James B. Sweitz Dump Trucking was not registered by the state of Oregon to do business in Oregon. At that time a call was placed to the applicant and it was agreed that the name of the applicant would be changed to M.E. Kauffman Quarries Inc. The application was then processed and a Note to reviewing agencies was drafted and circulated under this name. When DOGAMI requested the reclamation security there was a problem in getting it in the name of M.E. Kauffman Quarries Inc. At that time Tamara Sweitz wanted to get the reclamation securityin the name of James B. Sweitz Dump Trucking. In order to do that, DOGAMI requested a transfer application to convert the name on the pending permit back to James B. Sweitz Dump Trucking. In order to fulfill the transfer request Tamara Sweitz was informed that James B. Sweitz �· ·· . Dump Trucking had to be registered by the state of Oregon to do business in Oregon. SITE LOCATION This rock quarry is located approximately 1 mile south of Rose Lodge and is accessed from State Highway 18 via Slick Rock Creek Road. Follow Slick Rock Creek Road 0.6 miles south to the access road on the east side of Slick Rock Creek Road. SITE HISTORY Mining at this site dates back to at least the mid 1960's. Production of material at the site remained below the 5000 cubic yard threshold for any consecutive 12 month period. Production of material in excess of the 5000 cubic yard threshold for any consecutive 12 month period would have required a full operating permit. After contacting the operator of the site, JamesB. Sweitz Dump Trucking, and conducting a site inspection on April 6, 2006, DOGAMI determined that the operations at the site would soon exceed the 5000 cubic yard threshold for any consecutive 12 month period and that the landowner or operator needed to acquire an Operating Permit. DOGAMI received a complete application for an Operating Permit on August 11, 2006. The site will require a conditional use permit through Lincoln County. An application for a conditional usepermit has been submitted to and received by Lincoln County. SITE CONDITIONS initial site inspection was conducted by VaughnBalzer of DOGAMI on 4/06/2006. The following description is based An · on that inspection. This sidehill rock quarry is approximately 4-5 acres in size and has been excavated into a steep (1.5 H: 1 V) slope above Slick Rock Creek Road. The rock quarry is located in the northwesternportion of a triangular 11.40 acre tax lot (taxlot 700). Tax lot 700 is the proposed DOG AMIpermit boundary. A 1-2 acre processing and stockpile yard has been developed on the floor of the quarry. An excavated highwall along the western portion of the northern property line is located immediately adjacent to the adjoining ELM property. This highwall ranged in height from approximately 40 feet to the west to over 70 feet in the east. The western portion of this highwall is near vertical and unbenched. Two benches have been established on the eastern section of this highwall. An access road proceeds from the quarry floor to the east and then north to the working benches and the northeastern boundary of the site. A threefoot safety berm has been placed along theoutslope of the access road and the berm has been seeded. Approximately 2 feet of rocky overburden and topsoil exist above the rock resource. Minor amounts of this rocky overburden have been stockpiled in the safety berm along the upper quarry access road. Portions of the tax lot that have not been impacted by mining including a vegetated buffer strip along the road are vegetated with Douglas firtrees and other typical Coast Range vegetation.
