PRODUCT LIABILITY CUS MJ 802 (03/20) THIS ENDORSEMENT CHANGES THE POLICY. PLEASE READ IT CAREFULLY. EXCLUSION - SPECIFIED PRODUCTS OR SUBTANCES This endorsement modifies insurance provided under the following: PRODUCTS/COMPLETED OPERATIONS LIABILITY CLAIMS MADE AND REPORTED INSURANCE This insurance does not apply to any Claim or Suit arising out of the substances listed below. This insurance also does not apply to any Claim or Suit arising out of any product that contains, uses or incorporates any of the substances listed below. This exclusion applies to any Claim or Suit arising out of the manufacture, handling, advertising, distribution, sale, labelling, application, ingestion, consumption, testing, exposure to, or use of the substances listed below, whether as a separate ingredient or in combination with any other ingredient or substance in any product. This exclusion also applies to any Claim or Suit arising out of any derivative, combination of or extract of the substances listed below. 1. Accutane 2. Anabolic-androgenic steroids; 3. Anabolic steroids; 4. Androstenedione and any derivatives and all steroid precursors; 5. AMP, AMP Citrate, 1, 3-dimethylbutylamine citrate, 1, 3-dimethylbutylamine HCL, methylpentanamine, 2-Amino-4-Methylpentane Citrate, 4-Amino-2-Methylpentane Citrate, 4- Amino Methylpentane Citrate, Amperall, 4-AMP Citrate, 4-Methyl-2-Pentanamine 6. Aristolochia spp., Aristolochia, Aristolochic acids; Aristolochia fangchi, Akebia spp., Asarum spp., Bragantia spp., Clematis spp., Cocculus spp., Diploclisia spp., Fang Chi, Guang fang ji, Kan- Mokutsu, Menispernum spp., Mokutsu, Mu Tong, Fang ji, Sinomenium spp. and any adulterated botanicals, botanical derivatives or other products that contain aristoochic acid, aristolochic acid derivatives or aristolochic acid extracts. 7. Picamilon, pikatropin, pikamilon, nicotinyl-gamma-aminobutyric acid, nicotinoyl-GABA, and 4- (Pyridine-3-carbonylamino)butanoic acid. 35. Sildenafil, Tadalafil and Vardenafil 36. Silicone 37. Skullcap (scufellaria lateriflora) 38. Stephania, Magnolia or any adulterated botanicals 39. Swine Flu vaccine including any and all strain or strains 40. THG (tetrahydrogestinone) 41. Thalidomide 42. Tocopheryl Acetate 43. Vitamin E Acetate 44. Willow Bark 45. Yohimbe (Pausinystalia yohimbe) 46. Any and all heavy metals including but not limited to lead, cadmium, mercury, chromium, zinc, copper, nickel, iron, and arsenic 47. Kratom (Mitragyna speciosa) 48. Any product, supplement or additive determined by the United States Food and Drug Administration at any time to be a “Class I Health Hazard”. Class I Health Hazard means a product presenting a reasonable probability that the use of, or exposure to it, will cause serious adverse health consequences or death, excluding Cannabis.
