Grade ______ Brockton Public Schools invite you and your Summer Reading List and Log family to participate in our Summer Reading Program. This year’s theme is Reading Log “Be a Reading CHAMPION.” Parent or Guardian 2021 Title Initials Students entering grades Pre-Kindergarten and Kindergarten can read books from the Recommended Authors List, the Suggested Titles List or students may select a book of their choice. As a parent or guardian, you may pick books to read together, too. Just record the title of any books your child reads this summer on the reading log and then sign your initials. Look for the log on the back of this brochure or on our website: www.bpsma.org. Send this form back with your child in the fall for a certificate of participation!
