Eligibility Criteria: High risk seniors living in the community, 65 years and over. Referral Process: CCAC does all referrals and assessments, includes self-referrals and physician referrals. Assessment Process: Eligibility for this program is determined through a RAI CHA assessment conducted by the North East Community Care Access Centre.
