Eligibility Criteria: a. Registered APWD (Bldg) Class I(A)/(B)/(C) & Class II b. Contractors having completed works in any one year during last three years a tleast one similar nature of completed work value not less than 80 % of tender Value. c. Up to date Registration Certificate, VAT, PAN, work completion certificate with supporting work order, Labour licence etc. must be produced at the time of purchasing bid documents. d. The contractor who was earlier allotted any NRHM works by APWD (B), Assam or by NHM and could not complete the work in all respect in time will not be ELIGIBLE to participate in the bidding process. e. The contractor & to whom extension of time was granted from NHM imposing penalty will also not be ELIGIBLE to participate in the bidding process. f. In case the evaluated Bid Amounts are equal, preference for allotment of the project will be given to the bidder who has completed Hospital related Building works on earlier occasion. 2. Cost of tender document should be drawn in favour of State Health Society, Assam, payable at Guwahati in the form of Pay Order/Demand Draft. 3. Bid security with a validity of 225 (i.e. 180+45) days should be in the form of FDR/CDR/CD and drawn in favour of the State Health Society, Assam, payable at Guwahati. 4. Date for issuing of Bid Document: 18-01-2016 to 19-01-2016 upto 17-00 hours. 5. Last date and time of submission of Bid: 25-01-2016 at 14-00 hrs a. The bids will be opened at 15.30 hrs of the same days as mentioned above. b. In case, if any Bandh/Holiday falls in any sequence, the sequence will be held in the next working day. c.
