Traverse City, MI 49686 231-995-6111 Immediate Release: May 24, 2021 GTCHD weekly outreach clinics schedule May 24 - 28 The Grand Traverse County Health Department (GTCHD) continues to partner with community agencies and mobilize outreach clinics throughout the county to make the COVID-19 vaccine more accessible for all. In an effort to reach and hopefully surpass the State of Michigan’s goal of 70% vaccinated in our county, the following locations, dates, and times will offer walk-in availability for the COVID-19 vaccine. For the week of May 24 - 28, 2021, the following locations will be: Date: Monday, May 24 Clinic Location: Cherryland Mall (1700 S. Garfield Rd.) Time: 9:00 a.m. - 4:00 p.m. Vaccine offered: Johnson & Johnson (18 and older) and Pfizer (12 and older) Date: Tuesday, May 25 Clinic Location: Cherryland Mall (1700 S. Garfield Rd.) Time: 11:00 a.m. - 6:30 p.m. Vaccine offered: Johnson & Johnson (18 and older) and Pfizer (12 and older) Date: Wednesday, May 26 Clinic Location: Cherryland Mall (1700 S. Garfield Rd.) Time: 11:00 a.m. - 6:30 p.m. Vaccine offered: Johnson & Johnson (18 and older) and Pfizer (12 and older) Date: Thursday, May 27 Clinic Location: Cherryland Mall (1700 S. Garfield Rd.) Time: 9:00 a.m. - 4:00 p.m. Vaccine offered: Johnson & Johnson (18 and older) and Pfizer (12 and older) Date: Friday, May 28 Clinic Location: Cherryland Mall (1700 S. Garfield Rd.) Time: 9:00 a.m. - 12:30 p.m. Vaccine offered: Johnson & Johnson (18 and older) and Pfizer (12 and older) Any child that 12 to 15-years-old must be accompanied by a parent or guardian. Verification of date of birth is required. Anyone 16 to 17-year-old, must have a parent or guardian fill out a minor consent form and a vaccination administration record form for the minor to bring with them to the clinic. www.gtchd.org As of May 21, 56.7% of all Grand Traverse County residents aged 12 years or older have completed COVID-19 vaccination (two doses of the Pfizer or Moderna vaccine or one dose of the Johnson & Johnson vaccine).
