QO/ l- NOTICE INVITING TENDER Sealed application & tenders are hereby invited by the Secretary, District Health & Family Welfare Samity , Howrah from bonafide, registered firm, having valid sales taxes & income tax certificate and at least one year past experience in the similar nature of business for Spectacles to be supplied to the office of the 4 Blocks (Amta - 1, Bagnan - 1, Bagnan - 2, Unpur) , 2 Hospital (Gabberia SGH and Unpur SGH). The rate should be quoted in percentage basis, both in figure and in words Corrections, if any, shall be made by crossing out and rewriting with proper attestation. 6. Self attested copies of valid & up-to- date IT, PT, Trade License, VAT Return, PAN Card are to be attached with the bid. 7. Payment shall be made after receiving goods in good condition and Proper qualities & requisite quaquantities .. 8. Bills in duplicate along with recipient challans are to be furnished to the office of the undersigned for payment. 9.
