Mental Health Community Support Services June 2014 Fact Sheet 2: Information for Existing Clients Fact Sheet: Information for Existing Clients This fact sheet provides information for current clients of mental health community support services provided under the Victorian Psychiatric Disability Rehabilitation and Support Services (PDRSS) program. It includes information about the changes that will happen to these services over the next few months, when these changes will occur and how the changes might affect you. As part of these changes the Victorian Government will also change the name of the PDRSS program to Mental Health Community Support Services (MHCSS) program from August 2014. This change is about supporting you. The changes will ensure access to mental health community support services is fair and equitable by ensuring people most disabled by their mental health condition do not miss out on the support they need. The reforms will make it easier for new and returning consumers and carers to get help no matter where they live. This will be achieved by establishing a centralised intake point for all referrals in each local area. A fact sheet explaining how the new intake assessment system will work will be made available shortly. We also know you want services to be of a consistent quality and designed around your individual needs and preferences. This is why we are introducing Individualised Client Support Packages. They will replace standard, moderate and intensive Home Based Outreach Services, Day Programs, Care Coordination, Aged Intensive Program and Special Client Packages from 1 August 2014. Your support package will help you to develop the life skills needed to live well in the community and to manage your mental illness. You will also be supported in other areas of your life such as finding and keeping stable housing, developing relationships, engaging in community activities, improving your health and wellbeing, or finding employment. Page | 1 Department of Health Continuity of support From 1 August 2014, it is possible that the service provider/s in your local community may change. This will not affect your eligibility. As an existing client of a PDRSS program, you will continue to receive support. If you are 65 years of age or over at the start of the new service arrangements on 1 August, you will still be eligible to receive services. Many people will be able to stay with their current service provider. Some people, however, will need to shortly move to a new provider. Depending on where you live, there could be up to three service providers for you to choose from in your local area. You also might be able to choose a new provider in your local area if you would like. Even if your current service provider is not changing, you might notice some changes in the way the service operates. You are encouraged to discuss any questions you have with your current service provider. Some providers of Youth Residential Rehabilitation (YRR) service will also change. Please be assured that if you are a current resident of a YRR service you will continue to be supported. When will these changes begin? The new service delivery arrangements for Individualised Client Support Packages, Youth Residential Rehabilitation services and the new intake assessment point for new clients, will start from the beginning of August this year. From this time onwards, you will be allocated a support worker and will be able to start developing your support package. Support will be provided on an individual and/or group basis, depending on your preferences and the type of support you need. Who are the service providers in my area from August 2014? Contact details of the new mental health community support service providers that will deliver Individualised Client Support Packages in each local area are attached to this fact sheet. Information on the providers of Youth Residential Rehabilitation services and the new intake assessment and planning functions is available on the reform website: www.health.vic.gov.au/mentalhealth/pdrss-reform. Where can I get support and find out more information? You are encouraged to discuss these changes with your current service provider who will support you to move across to a new service provider in your area if required. Over June and July, transition information/navigation workers will also be available to help you choose a new provider in your local area and help you make contact with your new provider (if needed).
