Laishram Kiranmala Devi, aged 36 years, W/o, L. Sharat Singh of Khangabok Part-L, Thouobal District was brought dead to the District Hospital, Thoubal at 9.40 P.M. of 7th June, 2021; and Sangaiyumpham2. Whereas, theon 7thdeceased June, 2021person before was reportedlybeing brought administered dead to CoVID the District vaccine Hospital, at PHC Thoubal. 3. Now, therefore, the Governor of Manipur is pleased to appoint an Enquiry Committee with Dr. L. Ashananda, Sr. Regional Director, Ministry of Health and Family Welfare, Government of India and Dr. Kh. Sasheekumar Mangang, Additional Director of Health Services, Manipur as members to enquire into the matter relating to the death of (L) Smt. Laishram Kiranmala Devi. 4. Terms of reference of the Enquiry Committee shall be as follows: The Enquiry Committee shall establish the events relating to the COVID vaccination process adopted at PHC Sangaiyumpham and other related issues which the Committee considers relevant to the inquiry. (i) To establish possible cause of death of the deceased person. 5. The Committee shall submit a report within a period of 15(fifteen) days without fail and the Committee shall be at liberty to call the Doctors/Paramedical Staff and Officials of the Covid Vaccination Centre at PHC, Sangaiyumpham and the concerned Doctors and Staff of District Hospital, Thoubal for examination and recording their statements wherever necessary in connection with the inquiry. The Committee may also interact with other relevant persons or examine records as required.
