Students must be enrolled in or recently graduated from institutions that have Alpha Eta Chapters. Students must demonstrate exceptional service within and beyond their program of study and beyond the confines of their institution (community service) in addition to demonstrating membership in professional societies and leadership positions in those societies. The awardee a $1,000 honorarium and will be recognized at the Eta Annual Meeting which will be held virtually on October 30th, 2020. Each institution with an Alpha Eta Chapter can submit up to 2 nominees. To be considered, the faculty advisor from the discipline must submit: • Nomination Form • A letter from the nominee demonstrating: o Membership and leadership positions in professional societies o Achievements / accomplishments of the nominee that addresses service to: ▪ The nominee’s program of study ▪ To the college-wide institution, and ▪ To the community beyond the institution • A copy of the nominee’s resume • Letters of support from o President of the nominating chapter o Dean/Administrator of the allied health unit o Faculty / Advisor from the discipline Instructions for Submission: Please submit all documents as a single .pdf file by August 15th, 2020 with the following subject: Alpha Eta Professional Service Award: nominee name To: [email protected] .
