This Air Force Guidance Memorandum immediately implements changes to Air Force Instruction 36-3202, Separation Documents. Compliance with this Memorandum is mandatory. To the extent its directions are inconsistent with other Air Force publications, the information herein prevails, in accordance with AFI 33-360, Publications and Forms Management. 2. The Department of Defense has approved DD Form 214 format changes for the Air Reserve Component. The changed guidance is summarized in the attachment. 3. This memorandum becomes void after 1 year has elapsed from the date of this memorandum, or upon publication of an Interim change or a rewrite of the affected publication, whichever is earlier. Multiple periods of active duty by Air Force Reserve Component members ordered to active duty for a non-contingency/contingency operation are authorized to be documented of on a single DD Form 214 (Cumulative DD 214), with the following conditions: 1. The dates of all active duty periods being documented and the contingency operation supported during each active duty period must be listed in the DD Form 214 Block 18, Remarks, or on the DD Form 214C. 2. The type separation, character of service, separation authority, separation code, reentry code and narrative reason for separation must be identical for all of the active duty periods being documented on the single DD Form 214. 3. Issue a single cumulative DD Form 214 when a member is active duty in direct support of contingency operations upon accumulation of 90 days or more, member’s transfer to another component, separation, retirement, or by member’s request. 4. Issue a cumulative DD Form 214 when a member is active duty in support of non-contingency operations upon accumulation of 90 days or more, member’s transfer to another component, separation, or retirement. Separation occurs when members of the Air Force (AF) leave active duty, move from one active duty status to another, or are discharged, retired, transferred, dismissed, or released. This instruction implements Air Force Policy Directive (AFPD) 36-32, Military Retirements and Separations, 14 July 1993, Department of Defense (DoD) Instruction (DODI) 1336.1, Certificate of Release or Discharge from Active Duty (DD Form 214/5 Series), 6 January 1989; and portions of DoD Directive (DoDD) 1332.14, Enlisted Administrative Separations, 21 Dec 93, DoD Directive 1332.30, Separation of Regular Commissioned Officers, 21 Dec 93. The Privacy Act of 1974 affects this instruction. Title 10, United States Code (U.S.C.), Armed Forces, Sections 1168 and 1169, authorize the maintenance of the System of Records FO36 AFPC P, Separation Case Files, (Officer and Airman) prescribed by this instruction. A Privacy Act statement must be included on DD Form 214WS, Certificate of Release or Discharge From Active Duty. This publication does apply to the Air Force Reserve Command (AFRC) and the Air National Guard (ANG). Any organization may supplement this volume.
