Community Glaucoma Monitoring Scheme It has been decided that your eye condition is stable and therefore you can be discharged to the above scheme. The scheme is a community based on-going care service provided by accredited optometrists for patients discharged from the Royal Eye Infirmary with the following eye conditions: Ocular hypertension, Glaucoma or a glaucoma suspect What happens after discharge? Your first appointment will be 6 months after your discharge and then normally every 12 months. Once you have chosen your preferred optician practice, the practice will contact you in the month before you are due to book your appointment. If you need to be referred back to the Royal Eye Infirmary this will be undertaken by the optometrist. Choice of opticians See the choice of community opticians overleaf There are two ways you can inform the glaucoma monitoring scheme of your choice; either by calling 01392 834026 with your preferred optometrist practice or by sending back the cut off slip at the bottom of the letter.
