The service provides a range of support including support to people who live in their own homes or with family and or carers. The service operates with teams based in Mull, Iona, Tiree, Coll and Colonsay. The service's aims and objectives state: - To provide support for individuals and families in their own home with the aim of improving and maintaining maximum independence - To enable clients to exercise control over their own lives - To support the individual or family in their own home - To avoid the need to move to another setting - To provide a service which is responsive to service users taking account of their race, age, gender, and communication needs - To ensure an appropriate service which is responsive to service users' culture and religion and is delivered in an anti-racist way - To provide a service that is responsive to carers' needs - To support informal and other formal carers in caring - To provide a choice of practical and caring support to which people have easy access - To provide a service of high standard - To eliminate discrimination on unlawful grounds and to promote the concept of equality. What people told us The views expressed in the questionnaires we sent out were positive and indicated that the people using the service were receiving an individualised service which meets their needs. The people we spoke with during the inspection were very happy with the way staff supported them and some expressed anxiety about the ongoing process of change relating to the integration of health and social care. Inspection report for Mull & Iona, Coll & Tiree and Colonsay Home Care Service/Housing Support Service Inspection report Self assessment We received a completed self assessment document from the service provider before the inspection. We were satisfied with the way the service provider had completed this and with the relevant information they had given us for each of the headings that we grade them under. The service provider identified what they thought they did well, some areas for development and any changes they planned. The process of involving staff and service users in the self assessment process should be developed further. From this inspection we graded this service as: Quality of care and support 4 - Good Quality of staffing not assessed Quality of management and leadership 5 - Very Good What the service does well There was strong evidence that the people using the service and their families are involved in planning their support arrangements and are regularly consulted about the service they receive. Staff are supported to work in geographical teams and this contributes to consistency for service users. The local managers were able to evidence some very good practice in relation to care planning and multidisciplinary working. We also saw that staff were well motivated and showed genuine care for the people they support. We saw that some staff were using the Best Practice in Dementia Care training resource and that this was making a positive difference to staff and service users. We saw that the provider had a clear strategy for the implementation of the integration of Health and Social Care on the islands and that the external manager is very involved in the day-to-day management of the service. What the service could do better Detailed feedback was given to the external manager at the end of the inspection. This is a short summary of the findings of the inspection. Care plan documentation is being updated and improved and it will take some time to achieve consistently high standards across the service. There is an ongoing review of the service provided to those living in Bowman Court focusing on improving outcomes for service users and further promoting independence. There are ongoing issues about the administration of medication. We were informed that there will be changes to job descriptions which will allow staff to administer medication and received the appropriate training to achieve this. A requirement is made about this. (Requirement 1). Inspection report for Mull & Iona, Coll & Tiree and Colonsay Home Care Service/Housing Support Service Inspection report We saw that some people needed improved foot care and that there was some confusion about the role of staff in looking after people's fingernails. Care plans should include clear details about how these needs should be met and staff should have appropriate foot care training to allow them to meet people's needs. A requirement is made about this (Requirement 2). We found that staff were concerned about the major changes which are planned for integration - the external manager is working to mitigate the impact of change and ensure that staff receive good support during this process. Requirements Number of requirements: 2 1. Interim guidance and procedures relating to prompting service users to take their medication should be put in place as a matter of urgency to ensure the safety of service users and staff. This is to comply with: SSI 114 Regulation 4 (1) Welfare of Users (a) providers shall make proper provision for the health and welfare of service users. Timescale: 31/7/2017. 2. Staff should be given clear guidance and training in relation to foot care and the care of fingernails. This is to comply with: SSI 114 Regulation 4 (1) Welfare of Users (a) providers shall make proper provision for the health and welfare of service users. Timescale: 30/9/2017.
