VALUED EDUCATION WITHIN REACH The Archdiocese of Chicago wants to help you learn more about the new tax credit scholarship program and get a jump start on the application process at an education event near you. Help will be available in English and Spanish. Tax credit scholarships will be distributed by Scholarship Granting Organizations (SGOs), not the Archdiocese directly. These scholarships will be awarded on a first come, first served basis, so it is very important that when the application process begins, you apply early! Where: DePaul College Prep, 3633 N. Sangamon Street, Chicago, IL 60620 Date: Thursday, January 18 Time: 6 – 8 p.m. Where: Cristo Rey Jesuit High School, 1852 W. 22nd Place Chicago, IL 60608 Date: Thursday, January 18 Time: 6 – 8 p.m. At the education events we will have scanners and thumb drives available for you to scan in the documents you need to apply for a tax credit scholarship and save for future use when applying. Please see the other side of this page for materials you should bring to the education event for scanning. VALUED EDUCATION WITHIN REACH You will need to include copies of the following materials when it is time to apply, so please bring these documents with you to the education events above to make sure you have the right materials, and so you can scan copies for future use. We will have scanners and thumb drives available at the events for your use. Household income: Form 1040 (Federal Tax Return), pages 1 & 2 only, 2016 or 2017 Complete tax return required if a business owner If you do not file taxes, provide other evidence of income, like W-2, recent pay stubs, Form 1099, official letter from employer or disability statement Proof of residence (one of the following): Illinois Driver's License with current address Illinois State ID with current address Utility bill with name and current address Evidence of child’s age for kindergarten and 1st grade students only (one of the following): Birth certificate Passport Public assistance (provide documentation if you receive benefits for): SNAP WIC Families should know the number of the Public School District in which the household resides.
