The following amendments may be taken note of, prior to submission of bid. 1. “Evaluation of Technical Proposal” under the head “Evaluation of Bids” (Section-3) is amended to read as follows: Criteria/Parameter Marks Tally Maximum Marks 1.Experience of the Bidder/Consortium in implementing similar project MMU/ Ambulance Service at community level: a.) 5 years and above 10 10 b.) 3-5 years 5 c.) 0-3 years 3 2.) Clause No-2 (5) of “Obligation of the service provider” under the head “ Job Description ” (Section-4) of the bid document is amended to read as follows: “The Service provider shall follow the Service Plan/Route plan/Calendar for MMU as approved by the District Health officer/JDHS. It is expected that there will be at least 22 camp days and 2 non camp days in a month. Non camp days could be used for maintenance, refilling and data entry purposes. Sunday will be the week off day. However, the competent authority may declare any other day in the week as “off-day”. In exceptional circumstances, the weekly off day can be cancelled by the competent authority.” 3. Clause No -3 “Modification of agreement” under the head “Terms and Conditions ” (Section-6) of the bid document is amended to read as follows: “The agreement when executed by the parties shall constitute the entire agreement between the parties in connection with the jobs / services and shall be binding upon the parties. Modification, if any, to the agreement shall be in writing and with the consent of the parties. The agreement shall be valid for a period of 5 years from the date of signing of the same. In case the service provider fails to adhere to the rules, regulations or any of the terms and condition of the agreement or in case the service provided is considered to be unsatisfactory by the MMUSPA, the service provider will be asked to provide his response in writing within 15 working days to specific case of violations and unsatisfactory services. The MMUSPA would be free to cancel the agreement after considering the response of the service provider and recording the reasons for its decision.” 4. Clause No-4 (a) “Performance Security” under the head “Terms and Conditions ” (Section-6) of the bid document is amended to read as follows: “The successful bidder shall furnish a performance security in the shape of Bank Guarantee issued by a Nationalized Bank in favour of Tender Inviting Authority for an amount equal to 5% of the one year contract value. The Bank guarantee shall be as per proforma at Appendix ‘E’ and remain valid for a period, which is three months beyond the date of expiry of the agreement. This shall be submitted within 15 days (minimum) of receiving of Notice for Award of Agreement and before signing of the agreement failing which the EMD may be forfeited.” 5. Clause No-11 (a) “General Terms and Conditions” under the head “Terms and Conditions ” (Section-6) of the bid document is amended to read as follows: “The Service provider shall commence the proposed services within 90 days of signing the agreement” 6. APPENDIX –D (Financial Bid) shall stand replaced by APPENDIX – D (Rev-1) included in this document. 7. APPENDIX –H (District wise MMU Details) shall stand replaced by APPENDIX – H (Rev-1) included in this document. 8. ANNEXURE - III (Staff Composition) shall stand replaced by ANNEXURE – III (Rev-1) included in this document. 9. Details of existing staff working under MMU along with their existing salary are given at APPENDIX –I included in this document. 10. Minimum 2 vehicles shall be deployed for each MMU at all locations. 11. The number of units of MMU has been increased from 50 to 90 units. These additional 40 units will be deployed specifically in Tea Garden areas as indicated at APPENDIX –H (Rev-1). It is to be noted that the total number may be further increased by 40 units in Tea Garden areas during the contract period which dependent on approval of Govt. of India. Revised bidding schedule is as follows: Closing date & Time of Receipt of Bid : 8/11/2016 up to 2.00 Pm Date & Time of opening of Technical Bid: 8/11 /2016 at 3.00 Pm All other terms & conditions of the tender shall remain unchanged.
