Abstract Periodontitis is the microbial infection often causing inflammation of the gingiva, bone loss and tooth mobility. Apart from periodontitis, periodontal bacteria like Porphyromonas gingivalis, Spirochetes, Treponema denticola are known to cause systemic diseases such as cardiovascular diseases, preterm low birth weight infants, Alzheimer’s diseases etc. Alzheimer’s disease is a progressive mental deterioration of the brain that can occur in middle or old aged individual due to generalized regeneration of neurons in the brain. Literature has shown that inflammation plays a vital role in the progression of Alzheimer’s disease where periodontitis is a risk factor proving the focal infection theory. This article reviews the relationship of periodontitis in Alzheimer’s disease and its role in modifying the disease. Keywords: Alzheimer’s Disease, Chronic Periodontitis, Periomedicine, Inflammation two-way relationship exists between periodontal disease Introduction and systemic health or disease. Inflammation is known to be a protective response that Untreated periodontitis can alter various systemic focuses on the removal of stimuli responsible for damage diseases progression and one such is Alzheimer’s disease. to the tissues, thereby leading to restoration of health. Alzheimer’s disease is a progressive disease that Periodontitis is an inflammatory condition representing degenerates brain cell connection and the cells die the response of periodontal tissues to bacteria like eventually destroying memory and other mental Porphyromonas gingivalis, Treponema denticola and functions. Spirochetes. Periodontitis leads to inflammation of gingival and destruction of periodontal tissues. The Though periodontopathic bacteria are responsible for characteristics of periodontitis include microbial biofilm the occurrence of periodontal disease, the progression of formation, gingival inflammation, alveolar bone loss and disease mainly depends on host inflammatory response.3 attachment loss.1 In an healthy periodontium, a balance between bacteria and host response is well maintained, whereas in Periodontal medicine establishes a strong relation periodontitis, that balance is disrupted; as the between periodontal health or disease and systemic periodontal bacteria and their products enter the blood condition. Periodontitis as a source of inflammation stream from periodontal pockets, resulting in bacteremia contribute to exacerbate many systemic diseases like and dissemination of its products in systemic circulation, diabetes, cardiovascular diseases, respiratory diseases, following which innate and adaptive immune response pregnancy (PROM- Premature rupture of membrane), results in changes of local vasculature, generation of an neurological diseases etc. Hence, periodontitis is not inflammatory response and the secretion of pro- limited to destruction of periodontal tissues; but also inflammatory cytokines like Tumor necrosis factor- modify systemic diseases and vice versa.2 Thus, a proven α(TNF-α), Interleukin-1(IL-1) and Interleukin-6(IL-6).This 274 | P a g e Nivetha K et al. It is Poole et al have reported that Porphyromonas specifically important in non-proliferative cells, like gingivalis lipopolysaccharide have been found in the neurons where its failure leads to neurodegenerative brains of Alzheimer’s disease patients, supporting the diseases. Physiologically as cells are aging, the hypothesis that Porphyromonas gingivalis play an unnecessary proteins or those produced in excess that important role in the pathogenesis of Alzheimer’s alter cellular balance are eliminated rapidly.
