Annex 1 12 July 2021 Details of Compulsory Testing Notice A. Places visited by a locally confirmed case involving L452R mutant strain 1. Any person who had been present at Gin Curry, Shop G7, G/F, Temple Mall South, 103 Ching Tak Street, Wong Tai Sin, Kowloon, Hong Kong at any time during the period from 6:00 p.m. on July 8, 2021 to 0:00 a.m. on July 9, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by July 13, 2021. If persons subject to compulsory testing have previously undergone testing between July 9 and July 11, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 2. Any person who had been present at Sushi Express, Shop 261, 2/F, Hsin Kuang Centre, 120 Lung Cheung Road, Wong Tai Sin, Kowloon, Hong Kong at any time during the period from 2:00 p.m. to 6:00 p.m. on July 4, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by July 13, 2021. If persons subject to compulsory testing have previously undergone testing between July 7 and July 11, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 3. Any person who had been present at Eat Together, G/F, 8 Ngan Fung Street, Wong Tai Sin, Kowloon, Hong Kong at any time during the period from 6:00 p.m. to 10:00 p.m. on June 25, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by July 13, 2021. If persons subject to compulsory testing have previously undergone testing between July 9 and July 11, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 4. Any person who had been present at Hair Salon, G/F, 132 Kiu Kiang Street, Sham Shui Po, Kowloon, Hong Kong at any time during the period from 10:00 a.m. to 1:00 p.m. on July 9, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by July 13, 2021. If persons subject to compulsory testing have previously undergone testing between July 9 and July 11, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. B. Places visited by a non-locally confirmed case with L452R mutant strain 1. Any person who had been present at Wooloomooloo Steakhouse, UG17-18 & G7-8, Tsim Sha Tsui Centre, 66 Mody Road, Tsim Sha Tsui, Kowloon, Hong Kong at any time during the period from 6:00 p.m. to 10:00 p.m. on June 17, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by July 13, 2021.
