Cornett Acting Director</p><p>DATE: January 16, 2008</p><p>SUBJECT: Civil Rights Pamphlet</p><p>The Office of Human Resource Management (OHRM) has revised the Cabinet’s Civil Rights pamphlet. This new version of the pamphlet does not have a form number, but will be used instead of the Civil Rights Pamphlet (PAM-320), which becomes obsolete with the issuance of this transmittal letter. The pamphlet is available on the Cabinet for Health and Family Services (CHFS) Intranet Child Support Forms Site, as well as on the Division of Child Support (DCS) Internet Site.</p><p>As a result of this change, the Non-K-TAP Application (Form CS-33) procedural instructions and the Kentucky Child Support Handbook (KCSH) Subsection 14.050, Non-K-TAP Application Packet, have been revised. The CS-33 procedural instructions have been revised to update the documents provided in the application packet. Subsection 14.050 of the Child Support Handbook has been revised to include a link for both the English and Spanish versions of the new pamphlet.</p><p>This transmittal letter is to be saved and filed for future reference as it explains changes that have been made to the Prosecutor’ Handbook, Manual of Forms, Child Support Forms and the Kentucky Child Support Handbook..
