<p> Idaho Elks Rehab Local Lodge Grants Application</p><p>Idaho Elks Rehab Local Lodge Grants are intended to help local lodges address some unmet physical rehabilitation need in their community. They offer your lodge an opportunity to make a significant and meaningful difference in their community through an Elks-sponsored project.</p><p>Idaho Elks Rehab Lodge Grants are designed to set a standard of excellence in community rehabilitation through volunteerism, mentoring and service. Although lodges can partner with other organizations, Idaho Elks Rehab Lodge Grants cannot be used as a donation to another organization under any circumstances. However, they may be used to supplement a grant from another organization to have a greater impact. Successful Lodge Rehabilitation Grant applications combined with other organizations should have significant local Elks involvement. </p><p>Elk’s identification is also a key. We want the community and organization to clearly identify your local Elks Lodge with your project. Although Local Lodge Grants are not competitive, they must at a minimum identify a community need, address how the Local Lodge plans to address that need, and provide a plan to publicize that activity</p><p>To be eligible, this grant application must be submitted online no later than NOON Mountain Time on August 15. Late applications will not be accepted. Grants and accepted projects will be announced at the Idaho Elks Winter convention in November.</p><p>Before completing the application, please read the Rehab Grant Guidelines carefully and view the PowerPoint presentation found at www.Idahoelks.org. They are designed to help your organization design a project and complete an application with a higher chance of approval.</p><p>Finally, ALL PROJECTS MUST BE DISCUSSED ON THE LODGE FLOOR AND VOTED ON TO PROCEED.</p><p>For more information, please contact Idaho Elks Rehab staff @ 208-489-4660. Project Information</p><p>Background information: Identify the community need this project will address. [This is the case for why your lodge needs rehabilitation grant funds]. </p><p>Project Description: How will your lodge specifically address this community need? What will your Lodge do? How many volunteers will it need? If your organization has a partner in this project, we want to know what YOUR LODGE will do, not what the partner organization will do (Be specific, and demonstrate that your lodge has a plan. </p><p>Rehabilitation grants offer the chance to make a significant and meaningful difference in the community. What lasting impacts do you think the project will have? Plan for Success</p><p>In this section, please indicate how you will know whether your project is successful. For example, at the end of the year, how will you know the project has helped resolve the problem identified from the background information in the previous section? </p><p>Publicity This is an opportunity to raise your Local Lodge’s profile and inspire new volunteers to join the Elks. How does your plan promote this project as an Elks-sponsored project in your community? Be specific. At a minimum, your project should include press releases to your local paper and radio stations. Budget Information</p><p>The project budget is the estimated financial plan for your project. The budget should include all expenses you anticipate incurring throughout the grant period (June 15 through March 1). </p><p>Staff Salaries & Support: Idaho Elks Rehab funds cannot support the salaries of project staff, However, a matching share of in-kind or cash personnel costs is allowable, (e.g. volunteer time). </p><p>Equipment: Equipment is defined as non-expendable items with a value exceeding $200. This may include new or updated equipment, rental equipment or leased equipment essential to the project. Examples of allowable equipment may include power tools, computers and computer software. A detailed justification is required. Equipment is subject to Idaho Elks Rehab approval. </p><p>Materials & Supplies: Supplies may include consumable items that are essential to the project. This includes sports equipment, informational materials, tools, provisions, t-shirts or books. This list is not exhaustive. </p><p>Travel: Travel costs of individuals identified in the budget that are deemed reasonable and necessary to conduct project activities. Examples of travel include transporting project participants and volunteer mileage relating to project operation.</p><p>Other Services: Other Services may include stipends, reproduction/printing, telephone or any other service that does not fit into another category.</p><p>Cash IER In Kind Budget Category Donation Costs Funds Donations s Total Staff Salaries & Support Equipment: Materials & Supplies: </p><p>Travel </p><p>Other </p><p>TOTAL </p><p>*Note: The amount entered in the total "IER Rehabilitation Funds" is the total grant amount that you are requesting to support your project, which is not to exceed $1,000. </p><p>The total amount in "Total Project Cost" is the total amount that this project will cost, including the Rehabilitation grant, other funds and in-kind donations. The "Matching Share" should include all funds and donations that will be used in addition to any IER grant received. Budget Narrative</p><p>Please explain why each expense item is necessary for the project and how you propose to allocate funds for each line item over the funding period between June 15 and March 1. Be as detailed and specific as possible.
