<p> 2016-17 WEBSTER COUNTY 4-H HONOR AWARD APPLICATION MUST BE TURNED IN BY September 30, 2017</p><p>MEMBER’S NAME______AGE (as of Jan 1) ____</p><p>CLUB______How Many years as a member______</p><p>ONLY THE CURRENT YEAR POINTS COUNT. Current year starts October 1 and ends September 30 of the following year. No points may be accumulated from previous years! Please circle which one you are applying for.</p><p>CLOVER KID HONOR AWARD (ages 5 – 7) – Minimum of six items from club and county. Member five to7 years old as of January 1 of the current year enrolled. A Clover Kid Member must check six of the items, which includes the mandatory ones. Those qualifying will be award a 4-H t-shirt at Recognition Night.</p><p>JUNIOR HONOR AWARD MEMBER (ages 8 to 13) – Minimum of 12 items from club, county and district of which four must be under club and 4 under county. Eight to 13 years old as of January 1 of the current year. A Junior Member must be able to check 12, which includes the mandatory ones of the items to receive a t-shirt at Recognition Night. Must </p><p>SENIOR HONOR AWARD MEMBER (14 and over) – Minimum 18 items to receive a sweatshirt. Must be 14 years and older as of January 1 of the current year enrolled. A senior must be able to check 18 of the items listed, of which at least five from club, five from county and two under district and/or state, which includes the mandatory ones. Those qualifying will be award a sweatshirt at Recognition Night.
