<p> Application for: ARCHAEOLOGY EXCAVATION PERMIT Under Provisions of the Utah State Antiquities Act (Utah Code § 9-8-301 through 308, and R694-1) 1</p><p>Carefully read and answer each question. You may attach additional pages as necessary. All applications must include a copy of the research design, a 1:24,000 scale map of research location, and written permission from land owner(s) to enter and excavate on the property. The State of Utah grants excavation permits to PLPCO permitted Principal Investigators; permits are not granted to organizations or institutions. Box 141107 Salt Lake City, Utah 84114-1107</p><p>For assistance contact David Yoder at 801-537-9014 or [email protected].</p><p>1) Name, and the number of the applicant’s Utah state issued Archaeological Survey/Principal Investigator Permit.</p><p>2) Attach a research design that describes the nature of proposed investigations and clearly addresses the requirements outlined in Utah Code § 9-8-305(3)(a)(i): A) states the questions to be addressed; B) states the reasons for conducting the work; C) defines the methods to be used; D) describes the analysis to be performed; E) outlines the expected results for the plan for reporting; F) evaluates expected contributions of the proposed work to archaeological or anthropological science; and G) estimates the cost and the time of the work that the principal investigator believes is necessary to provide the maximum amount of historic, scientific, archaeological, anthropological, and educational information. </p><p>1 3) Provide the location of the investigation using site numbers, legal descriptions, land ownership, plot the location on a 1:24,000 scale map and attach a copy of the map.</p><p>4) Describe the nature of your expected reporting</p><p>5) Field work to Begin __ End ______</p><p>6) Please include any other information you feel is relevant to the processing of this application.</p><p>All applications must include: a complete research design or approved data recovery plan, written permission of the land owner to conduct excavation and a 1:24,000 scale map of excavation area(s).</p><p>Signature: Date: </p><p>By signing or submitting this application, the applicant certifies that the information contained within the application to be true to the best of her/his knowledge.</p><p>2</p>
