<p> Hazel & Theodore Johnson Scholarship Fund</p><p>Qualifications:</p><p>1. The student must graduate from Norwood High School and be admitted to attend the University of Cincinnati main campus or any branch campus of the University. A COPY OF YOUR ACCEPTANCE LETTER MUST BE ATTACHED</p><p>2. Parents of the student(s) must have financial difficulty in sending the child to college without financial assistance as evidenced by a federal tax return. A COPY OF YOUR PARENTS TAX RETURN MUST BE ATTACHED</p><p>3. The student and parent must have completed FAFSA A COPY OF YOUR FAFSA FORMS MUST BE ATTACHED</p><p>4. The student applicant must submit a written statement setting forth his or her goals in education and in life endeavors</p><p>5. The student must submit a resume of his or her education and accomplishments</p><p>6.
