If other arrangements need to be made, please email the registrar at [email protected] or the test chair at [email protected].</p><p>Note: If your test/critique requires 3 judges, you must pay a one-time additional fee of $75. </p><p>• Test fees are non-refundable. • Test Schedule will be posted, on the Treadwell rink the day of testing and sent via email to testers approximately 1 week before testing.</p><p>LIABILITY WAIVER AND INFORMATION ACKNOWLEDGMENT The USFSA and JFSC undertake no responsibility for damages or injuries suffered by the candidates. As a condition of and in consideration of the acceptance of their applications, all candidates and their parents and/or guardians shall be deemed to have agreed to assume all risks of injury to the candidate’s person and property resulting from, caused by or connected with the conduct and management of the tests, and to release any and all claims which they may have against any officials, the USFSA, the JFSC and against their officers. Applications shall be accepted only on the foregoing conditions.</p><p>I understand that incomplete applications will be returned.
