<p> Notice regarding Missed Visits</p><p>A Communication Note form is used to document a missed visit. However, if you meet the minimum weekly visit requirement from the Plan of Treatment then a communication note is not needed. For example, if the patient’s Plan of Treatment states one to two visits a week and two are scheduled but only one visit is made, then a communication note is not needed. Only when you do NOT meet the MINIMUM weekly requirement would a note be required.</p><p>Use one Communication Note for each patient and record all the missed visits for the month. If there is MORE than one missed visit, place the Communication Note on top of your daily visit notes for the month arranged in order with most recent note on top. If you have ONE missed visit for the month, then collate by date with the other daily visit notes.</p>
