Why did you choose a Labrador or Golden retriever as your breed of choice? </p><p>2. Have you ever owned a dog and been responsible for his/her care, training , etc?? If not still living with you, what were the circumstances surrounding his/her departure? 3. Are you aware of the financial commitment that a puppy will entail? For example, vet costs and dog food costs, crates, illnesses or accidents?</p><p>4. Type of dwelling? home , condo , apartment , mobile home , other</p><p>5. Do/will you have a veterinarian? Please give the vet’s name , address , and phone number. We often times use your veterinarian as a reference check.</p><p>6. Will the puppy share a home with others beside yourself? </p><p>7. If you have any pets currently, have they been neutered or spayed ?Is it your commitment to spay or neuter your puppy? (Not recommended until 18 to 24 months for health reasons) 10.
