<p> ACU Sub-Recipient Monitoring Plan Site Visit Checklist</p><p>Pre-visit</p><p>Determine whether other monitor(s) will accompany you on site visit, and if so, whom. Select site visit date with agency and other monitor(s). Make hotel and car reservations (if applicable). Choose at least two different months Contract Expenditure Reports (CERs) to review. Photocopy CERs and MERs for the chosen months. Choose at least ten client files to review from CAREWare (select from different services). </p><p>Make copies of the Client Chart Review Form (one for each client file chosen). </p><p>Make copies of programmatic and financial checklists and email checklists to agency. Send the QA packet (includes the Client Chart Review Form, programmatic and financial checklists and ACU Record Review Form) electronically to agency at least 24 hours in advance of site visit. </p><p>Review and bring prior high-risk letter to site visit (if applicable). Review and bring prior site visit report(s) if there are any findings/corrective actions that need to be followed up.</p><p>Bring entire agency CER folder. Secure directions to agency. Bring laptop, paper, a pencil/pen, and a calculator.</p><p>Post visit</p><p>Return car (if applicable). Complete travel reimbursement form and attach receipts (gas, car, and lodging). Give draft of site visit report to Unit Manager for review. Mail approved report to agency and distribute copies within three weeks of the site visit.</p>
