I am extremely disappointed at the fact that the Government has refused to address the very serious concerns raised by Senior Medical Officers across the State regarding the deficiencies of the proposed contract and the likely dire consequences to the public health system should the Government implement the contracts.</p><p>I attach my letter of resignation from the [insert HHS], which sets out my concerns regarding the contract. </p><p>I authorise ASMOF to act as my agent in tendering this resignation. ASMOF is authorised to tender this resignation on [insert date]. </p><p>I appreciate that this is a very significant decision for me to make and I understand that the result of this course of action will be to terminate my employment with Queensland Health three (3) months from the date my resignation letter is tendered unless both my employer and I agree otherwise. I also acknowledge that Queensland Health may accept this resignation and not require me to work my notice.</p><p>I am making this decision as I believe that the contract significantly reduces my present conditions of employment and will have a damaging effect on the provisions of high quality medical services to the Queensland public. I believe that this is the only option left to me to convince the Government that it must make changes to this flawed contract to guarantee fair working conditions and ensure Queenslanders have the best possible health system.</p><p>I ask that you confirm with me prior to submitting the resignation that I still wish to proceed.
