To do this, Manitoba Health, Healthy Living and Seniors is encouraging more doctors and nurse practitioners to work in the province. Several initiatives have also been introduced to ensure patients receive continuity of care. Such initiatives include integrated multi-disciplinary teams, like My Health Teams and the Family Doctor Finder program.</p><p>FAMILY DOCTOR FINDER</p><p>The Manitoba government is moving forward to ensure that every Manitoban who wants a family doctor or nurse practitioner will have access to one by the end of 2015. </p><p>The Family Doctor Finder supports these efforts by providing a much more customer-focused and active service to connect Manitobans who need one with a primary care provider than was previously available. When someone registers with the program staff in each health region called Primary Care Connectors now work with them to find a primary care provider in a convenient location. </p><p>To date, the program has matched about 25,000 registrants, who did not have one, with a family doctor or nurse practitioner since it launched as a provincial pilot in July 2013. </p><p>Anyone living in Manitoba who needs a family doctor can register with the program online at www.manitoba.ca/health/familydoctorfinder or by phoning 204-786-7111 (in Winnipeg) or 1-866-690-8260 (toll-free). </p><p>MY HEALTH TEAMS</p><p>My Health Teams (MyHT) are the main driver in developing a sustainable primary care system in Manitoba. A MyHT includes a regional health authority, fee-for-service physicians and community organizations that collaboratively work together to plan, deliver and improve primary care services. MyHT goals are to:</p><p> improve access to primary care for all Manitobans, and in particular, those without a regular primary care provider;</p><p> demonstrate quality and safety in primary care;</p><p> strengthen the emphasis on comprehensive patient-centred care including chronic disease management;</p><p> enhance continuity in primary care; and</p><p> build a co-ordinated primary care system, contributing to the sustainability of Manitoba’s broader health system.</p>
