<p> Cochrane Jaguars Volleyball Club Liability Waiver 2017/18</p><p>RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT. BY COMPLETING THIS DOCUMENT YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. PLEASE READ THIS CONTRACT CAREFULLY</p><p>Group Organizer's Name: Cochrane Jaguars Volleyball Club, hereinafter to be referred to as (" CJVC ")</p><p>ASSUMPTION OF RISKS: I am aware that having my child or legal ward: ______participating in the activities and sports, without limitation, offered by or associated with CJVC, exposes my child or ward named above to inherent risks, dangers and hazards. By engaging in any activities offered by or associated with CJVC, I freely accept and fully assume on behalf of my child or ward, all inherent risks, dangers and hazards and the possibility of personal injury, death, property damage or loss resulting there from. RELEASE OF LIABILITY WAIVER OF CLAIMS & INDEMNITY AGREEMENT: In consideration of CJVC permitting my child or ward to participate in its activities and sports, permitting my child or ward to the use of its equipment and permitting my child or ward the use of its facilities or facilities it agrees to rent for the above stated purposes, I hereby agree: 1. TO WAIVE ANY AND ALL CLAIMS that I have on behalf of my child or ward, or may in the future have against CJVC, and its directors, officers, employees, agents, representatives, volunteers, assigns and successors. 2. T0 RELEASE CJVC, and its directors, officers, employees, volunteers, agents, representatives, assigns and successors from any and all liability for any loss, damage, injury or expense that my child or ward may suffer or that my next of kin may suffer, as a result of my child’s or ward’s participation in activities and sports offered by CJVC, due to any cause whatsoever INCLUDING NEGLIGENCE, BREACH OF CONTRACT, BREACH OF STATUTORY DUTY OF CARE, AND/OR BREACH OF THE OCCUPIER'S LIABILITY ACT, R.S.A. 2000, C. O-4, ON THE PART OF CJVC and its directors, officers, employees, volunteers, agents, representatives, assigns and successors. 3. TO HOLD HARMLESS AND INDEMNIFY CJVC, and directors, officers, employees, volunteers, agents, representatives, assigns and successors from any and all liability for any property damage or personal injury to any third party, resulting from my child’s or ward’s activities and my child’s or ward’s participation in the activities offered by or associated with CJVC. 4. That this Agreement shall be effecting and binding upon any heirs, next of kin, executors, administrators and assigns in the event of my or my child’s or ward’s death. 5. I have read and understood this Agreement prior to signing it.
