codes with bleeding definition category of ‘other’ but also requiring a blood transfusion (ICD-9-CM procedure code 99.0 or Current Procedural Terminology [CPT] code 36430) within 7 days of the bleeding diagnosis or requiring hospitalization. †To identify hemothorax in the expanded ISTH definition ICD-9-CM code 511.8 (other specified forms of effusion, except tuberculous, including hemothorax) was linked with CPT code 32020 (tube thoracostomy with or without water seal); depending on whether fluoroscopy was used during the procedure, CPT code 76003 (fluoroscopic guidance for needle placement) was also linked. ‡In the expanded ISTH definition ICD-9-CM code 608.83 it was ensured that patients did not have an associated code for myocardial infarction, stroke, or ischemic event to exclude arterial thrombosis.

Analyses of VTE incidence were performed using three different definitions: - Definition A, was an ‘all inclusive’ analysis comprising all individuals with an ICD-9-CM code relating to VTE who met the inclusion and exclusion criteria for the study (i.e. study period, tumor location). This definition considered a VTE case as a patient who had at least 1 VTE claim. - Definition B considered a VTE case as only patients who had 2 or more outpatient claims at least 30 days apart, or one inpatient claim, or a single outpatient claim in which an anticoagulation drug was disbursed within 90 days, and excluded the ICD-9-CM code 997.2 (peripheral vascular complications not elsewhere classified). - Definition C, the most stringent definition, further excluded from definition B any VTE events within 90 days of any major or invasive surgery. Codes relating to esophageal, kidney, and uterine cancer were not included.

22 Figure S1. Study cohort selection.

23 Figure S2.
