<p> DUNLAP ADOPTION ASSISTANCE FUND GRANT APPLICATION</p><p>INTRODUCTION</p><p>The Board of the Dunlap Adoption Assistance Fund is excited about your desire to adopt! </p><p>Funds may be awarded in good faith in advance of a home study with the understanding that ARPC pastors and their families often need these funds up-front to begin the process.</p><p>Families must have concrete estimates on their initial adoption costs and fees prior to their commitment. Initial costs for a home study are approximately $3000 and at least $5000 for International adoptions. </p><p>Complete the DAAF Grant Application Agreement to indicate your agreement to the terms, conditions, and requirements of your grant application. The grant may be used for adoption expenses, including but not limited to, a home study for an agency adoption and/or legal costs of a private adoption. Yes No</p><p>If you answered yes to (5) or (6), please detail:</p><p>Section 2 – Grant Funds</p><p>(a) Applicant may use funds for any adoption related expense like home study, legal, or travel.</p><p>(b) Applicant acknowledges that The Dunlap Foundation Board retains full discretion to approve the Grant application.</p><p>(c) Applicant hereby releases the Dunlap Foundation and Board from any responsibility to approve the Grant application and any consequence or damage that Applicant might incur if the Grant is not approved, including but not limited to, financial or emotional distress or any impact of any nature on Applicant’s adoption. In no event shall the Dunlap Foundation be liable to Applicant for consequential, incidental, punitive, special, exemplary, or indirect damages or expenses, even if Applicant was advised of the possibility of the occurrence of such damages.</p><p>(d) Applicant acknowledges that the Grant shall not be deemed to have been approved until Applicant receives an official DAAF Approval Letter by email.</p><p>(e) If approved for an international Grant, Applicant agrees that the Foundation will disburse the Grant only after Applicant’s dossier has been submitted and Applicant has been invited by foreign officials to travel. All other adoption costs prior to approval, such as initial agency fees, dossier translation and submission and USCIS, remain the sole responsibility of Applicant and Applicant agrees to timely pay such costs, prior to commitment, to avoid delay of the adoption.</p><p>Section 3 – Donations/Family Sponsorship Program</p><p>(a) If the ARP Foundation receives funds designated for your adoption account which are not used, the balance of funds are subject to the Foundation’s rules and standards and are not refundable to the </p><p>Page 3 of 4 donor, but may be transferred to other adoption accounts, or be reallocated to another adoptive parent account.</p><p>(Section 3 – Continued)</p><p>(b) Applicant acknowledges that neither the Dunlap Foundation nor the ARP Foundation holds no responsibility to raise funds on behalf of Applicant, or Applicant’s adoptions. Applicant acknowledges that any fundraising on behalf of Applicant shall be the sole responsibility of Applicant.</p><p>(c) In the event of a successful adoption by Applicant, Applicant hereby grants permission to The Dunlap Foundation to appropriately use any photos of the family and children from your own public blog, Facebook posts, etc. as updates to your profile page on our website. </p><p>Section 4 – Confidentiality</p><p>(a) All communication and correspondence between Applicant and the Dunlap Foundation, the contents of this Agreement and the Grant application shall not be disclosed to the public or any representative of the Applicant. </p><p>(b) Applicant agrees and promises, on behalf of themselves and their representatives, not to engage, now or in the future, in any disparaging conduct directed at the Dunlap Foundation or its officers, directors, or volunteers.</p><p>Acceptance of Agreement Terms and Signatures;</p><p>By signing this Agreement below, we hereby verify the contents as accurate and submit this Agreement and Grant application upon these terms and conditions. </p><p>This ____ day of ______, 20____. </p><p>Applicant Signatures: MOTHER______FATHER ______</p><p>Page 4 of 4</p>
