I understand that I will always be accountable for my academic performances in this class. </p><p>______2. I am informed that tutorials are given every day from 2:45-3:15in case I needed extra help in this subject and I shall come voluntarily.</p><p>______3. I understand the grading criteria for this subject. </p><p>______4. I understand that if I miss a lab day, it is my sole duty to make it up two to three days it has been performed or my score will be forfeited. However, if in case I can’t do it for some important reasons, I shall notify my teacher about it and compromise for another day.</p><p>______5. I understand that retests or missed tests can only be taken after 3 days after the test has been given. Missed tests and retests are given in the afternoon of the stated day between 2:45-3:30 in the afternoon.</p><p>______6. I understand that I can ask for a special project in case I needed extra credits for my grade.</p><p>______7. I understand that I can also earn extra credits through active participation in the class, however, the points with it is determined by my teacher. Extra credit earned through recitation can’t be used during midterm and final tests.</p><p>______8. I understand that my lecture notebook is the primary requirement for this class.</p><p>______9. I understand that it is my sole responsibility to do make up works in the event that I missed the class for some important reasons. Failure to make up the missed works within a week would mean deprivation of any score.</p><p>______10. I understand that late requirements or projects will not be accepted. </p><p>B. BEHAVIOR</p><p>______11. I understand that I alone am responsible for my behavior and academic standing in this subject.</p><p>______12. I understand that I should come prepared to this class with all my materials primarily a lecture notebook and a writing material.</p><p>______13. I understand that when my teacher starts talking I should pay attention and shall raise my hand in case I would like to say something and wait to be acknowledged. ______14. I understand that the following procedures will be followed for any misbehavior; 1st verbal warning, 2nd Teacher’s D hall and third Office Referral. Automatic referral stipulated in the handbook shall also be followed strictly. However, severe misbehavior can be addressed directly by a teacher’s D hall or an office referral. </p><p>______15. I understand that I should not include myself in any discussion especially when my teacher is reprimanding any of my classmates.</p><p>______16. I understand that I shall have two hall passes for each 9 weeks and can only be used in such duration.</p><p>______17. I understand that any misbehavior not covered by these procedures shall be defined by the student handbook.
