<p> Peace Garden Consortium of Student Support Services</p><p>PGC 511</p><p>REPAIR/MAINTENANCE AGREEMENT For ADAPTIVE EQUIPMENT</p><p>1. The Peace Garden Consortium of Student Support Services owns a wide variety of materials that are available for schools, teachers, and parents to borrow. This includes software, assistive technology devices, positioning devices, and other items.</p><p>2. To borrow any of these materials, contact must be made through the PGCSSS office and the item will be checked out to you. The care and maintenance of this item then becomes the responsibility of the borrower. </p><p>3. If an item is not working properly when you initially borrow it, please notify the PGCSSS office immediately.</p><p>4. During the lending period, any necessary maintenance or repair, including batteries, paper, cartridges, cords, earphones, etc. is the responsibility of the borrower. </p><p>5. Items are expected to be in good working order upon return to the PGCSSS office. If they are not, the office may choose to secure the needed maintenance and/or repair and then may bill the borrower. </p><p>IMPORTANT…….PLEASE READ: Please set up the equipment/item you borrow within 5 days of receipt. Make sure you have received all the needed items. It was in good working order when taken from the PGCSSS office. If the item is not working properly, contact the office immediately. Upon return of the item, the borrower agrees to repair or replace any missing or damaged parts. Further, if this loan includes SOFTWARE, the borrower agrees to utilize this software for exploration of its features ONLY and will REMOVE it from their personal or school computer upon return of the item to PGCSSS.
