<p> Medical City Dallas Hospital located in Dallas, TX, is currently seeking an experienced Infectious Disease Pharmacy Specialist.</p><p>The role of ID Pharmacy Specialist with Medical City Dallas Hospital is key to the delivery of high quality care. Major job responsibilities for this role include:  Serves as the Subject Matter Expert for the facility: o Leads and mentors all aspects of the antimicrobial management program (AMP) and other clinical programs o Works closely with facility pharmacists to interpret prescribing and microbiological trends o Conducts facility specific medication use evaluations (MUEs) to determine prescribing and microbiological trends o Develops institutional guidelines for the management of common infection syndromes. o Prepares and updates facility specific protocols and procedures o Updates antibiogram, empiric guidelines and duration of therapy recommendations on an annual basis o Provides facility expertise on streamlining or de-escalation of drug therapy, dose optimization, and IV to PO conversion o Provides expertise on pharmacodynamic (PD) and pharmacokinetic (PK) dosing, appropriate utilization protocols, drug restrictions, and antimicrobial management program (AMP) initiatives  Infectious Diseases Education and Support:</p><p> o Facilitates facility pharmacist involvement with division AMP education</p><p> o Communicates with DDIDP and DDCP educational needs for self and facility</p><p> o Educates pharmacy staff on all AMP clinical protocols/policies prior to, during, and after implementation</p><p> o Works with the Pharmacy Clinical Manager/Coordinator to prepare and ensure yearly pharmacist competencies on AMP areas of focus are conducted and completed by pharmacy staff as necessary</p><p> o Facilitates, coordinates and/or conducts AMP education for nursing, physicians, prescribers, Infection Prevention, microbiology, or other facility staff </p><p> Development of AMP Program:</p><p> o Conducts gap analysis of the antimicrobial stewardship program</p><p> o Develops and implements a strategy for expanding and improving AMP at the facility</p><p> o Works in conjunction with the AMP physician to develop and implement AMP initiatives at the facility o In conjunction with the AMP physician, develops a vision as well as short and long term goals for AMP at the facility</p><p> o Conducts follow up to ensure newly implemented strategies are having the desired effect and meeting established goals</p><p> Patient Review:</p><p> o Responsible for development of processes within the facility such that all patients on antibiotics are reviewed for appropriate selection, dosing, and duration of therapy based on targeted HCA, division, and facility goals</p><p> o Develops and ensures that appropriate antibiotic renal dosing is performed </p><p> o Develops and monitors appropriate IV to PO antimicrobial conversions are performed</p><p> o Develops and ensures that appropriate criteria for use on antibiotics are followed</p><p> o Regularly meets with the AMP physician champion to review more difficult situations</p><p> o Develops positive relationships with key physicians and leaders.</p><p> o Regularly meets or discusses AMP-identified issues with other pharmacy staff</p><p> o Develops and ensures that appropriate recommendations are made regarding empiric therapy, duration of therapy, culture and sensitivity mismatch, de-escalation and other antibiotic stewardship interventions</p><p> Formulary Management and Dose Optimization:</p><p> o Performs appropriate drug class reviews as scheduled on the division formulary review calendar or as needed by the facility</p><p> o Develops and monitors processes are in place for adherence to formulary, including PD and PK dosing of antimicrobials </p><p> o Reviews non-formulary usage of antimicrobials </p><p> o Develops and monitors non-formulary use is appropriately indicated.</p><p>Education/Licensure Requirements:</p><p> Bachelor’s degree in Pharmacy required.
