Sponsor's SSN, Name, DOB e. Address for Delivery f. Patient's home phone # g. Attending Physician h. Peds Clinic Office Number (301-295-4941 or 202-782-6101) and fax (301- 295-6173 or 202-782-0740) </p><p>3. Enter CON order in CHCS for P-DME a. in comments section enter name of agency, the info above, and expected length of treatment </p><p>4. Write a Doctor's Order on Doctor Order sheet or on a Prescription Pad and fax to agency </p><p>5. Provide patient with name, phone number, POC and estimated time of delivery</p>
