<p>Principle Statement Heantun Housing Association is committed to provide a professional, equitable and service user focussed service to both those individuals requiring support services, and to those agencies making referrals on their behalf.</p><p>Heantun will ensure it operates, manages and monitors an effective, equitable, confidential and efficient referral and application process, throughout all of its support services, through the following areas; ● Responses to enquiries, referrals and applications ● Information and documentation ● Comprehensive assessment ● The decision making process ● Quality assurance</p><p>Service Information Further information on our Walsall Learning Disability support service including types(s) of support offered is available by contacting staff on . </p><p>Comprehensive Assessment All applicants will be required to undergo a formal assessment of their needs, clarifying any risk assessment information may also be required as part of the application process.</p><p>There is an expectation that the referral form must be completed in full prior to an assessment taking place. Other relevant information on the applicant requiring support service is recommended (if applicable) such as:- ● Risk assessment/information ● Medical report ● Social history report</p><p>Applicants and their referral agent will receive confirmation of their assessment in writing.</p><p>Service Eligibility Criteria ● A Walsall Borough resident or clear link to the Walsall Borough ● Aged 18 and over ● Has learning disability/difficulty (formal or informal diagnosis) ● Having clear identifiable support need/s</p><p>Walsall Local Eligibility Criteria</p><p>1 Walsall MBC adult services funding is used solely to carry out eligible support tasks. 2 These tasks must be shown to be part of an agreed support plan specifically aimed at promoting/maintaining independence which are preventative for users to have a meaningful life. 3 The support provided contributes to the person’s ability to live a meaningful life and/or participate in the life of the community, 4 The support provided is not exclusively focused on one aspect of that person’s life.</p><p>Reviewed 09/2016 The Decision Making Process The information provided on the application form and all other relevant documentation (if provided) is designed to enable Heantun staff to undertake effective assessment of person centred individual need/s that will provide the necessary information required in the decision making process.</p><p>All completed applications will be discussed at weekly allocations meeting/discussion and the outcome of each assessment is a decision to accept, refuse or defer the application with recommendations. Each application will be assessed on individual circumstances.</p><p>Successful applicants will be notified the offer of support service, or information about the service waiting list (if applicable). Unsuccessful applicants are informed of the reasons for rejection and where appropriate are signposted to other services.</p><p>Right of Appeal Any applicant who has been unsuccessful and feels they have not been treated fairly or in accordance with the organisation’s applications and assessments policy will have recourse to appeal. A letter of appeal, clearly outlining reasons should be sent to the service manager within 14 days of receiving the refusal letter and he/she will review the decision made.
